  2. Introduction: • By traditional treatment we mean those that are delivered from an institution in the form of inpt. or outpt. Care , alternative provision can be divided into institutional and community based models.

  3. Introduction: • Although it is possible to treat most pt. in alternative community setting , there are pt.s who in certain settings and under certain circumstances require more traditional in pt. Care

  4. Introduction: • In assessing the effectiveness of alternative models one has to be careful to examine whether it is more effective medication administration and prescribing practice that is creating an advantage for the alternative system of care.

  5. Introduction: • Another important aspect of the success of such programs is the quality of the personal involved and their knowledge experience and supervision, it is no coincidence that successful programs use more psychiatrist time or use the time available to better purpose.

  6. Introduction: • Providing a single institutional or community solution is clearly going to be wrong in a lot of cases because it will be appropriate and successful from some pt. and not others. A range of approaches is needed and getting the right pt. into the right setting is then the challenge.

  7. Introduction: • In this chapter I assume that by traditional treatment we mean those that are delivered from an institution in the form of in pt. or out pt. care in some services day hospital programs are well established and others various forms of partial hospitalization occur.

Introduction: • The penetration of partial hospitalization program varies from place to place and it is probably true to say that they are not commonly regarded as traditional approaches yet so in some settings a day hospital might be regarded as a radical alternative whereas in other a day hospital has operated from many years and is no linger regarded as either radical or alternative.

The extent and nature of evidence • There is good clinical evidence and ever some theoretical evidence dating back many years on which to bass judgment about alternative forms of treatment and care ( warner 1995) argues that non-institutional residential settings produce different results because:

The extent and nature of evidence People are called on to use their own inner resources they must exercise a degree of self control and accept responsibility for their actions and for the preservation of their living. Environment, consequently, pts retain more of their self-respect their skills and their sense of mastery the domestic and non - coercive nature makes human contact with the person in crisis easier than it is in hospital.

  11. Alternative residential treatment for acutely ill pts. • My intention here is to point out some of the key features of these treatment models rather than to describe then in detail:

  12. Cedar House Colorado , USA Cedar house is a 15-bed , 24 hour residential home in a house in the community; it aims to function as an alternative to psychiatric hospital for acutely ill pt. Staff and pt. share household duties, and come and go freely. The length of stay is brief about one or two weeks pts are discharged to suitable living conditions in the community, two staff members are always on duty the weekly costs of provision are much lower than hospital bed.

Crossing Place, Washington DC, USA Crossing place was established in 1978 and adopts psychosocial approach that reflects the significance of ethnic identity of staff and pt. the environmental realities of their lives, and the political and economical climate (Bourgeois 1995) the service aims to resolve crises in supportive milieu operating as a temporary family, clients served are over 18 years of voluntary status have no serious complicating medical problem other than substance misuse . suicidal and assaultive pts are not excluded. One-to-One intensive inter personal support is provided by specially trained staff in a home- like environment. A psychiatric evaluation is made within 24 hrs of admission about 60% of client have schizophrenia. About 10% of pt. have to be transferred to hospital for treatment.

  14. Comment… • Comparisons of these alternative services and standard services need to include all the pts they intended to treat or attempt to mach carefully the pts who are accepted for care.

  15. Residential alternatives forsever mental illness. • Northwest Evaluation Center. • An example of an alternative service for some of the most difficult pt is the Northwest Evaluation Center . • Involuntary detained pt. are held in locked residential treatment facility for up to 72 hour in the first instance and then for a further 90-180 days excluded pt under 18 yrs this center requiring 24-hour armed guard there are four psychiatrist who work in 24-hour segments. • Even though this also transfers 10% of pt. to the state mental hospital.

Non-traditional ( less conventional) services for ill patients: • Soteria Berne • Soteria Berne is based on integration of psychosocial and biological factors under medical supervision , it was opened in 1984 in a 12- room house in Berne for people aged 17-35 with a recent on set of schizophrenia or schizophreniform psychosis the most acutely disturbed pt may by pass the system altogether by going from the emergency room directly to hospital bed. • There are two staff continuously an duty and apart-time medical director. Five psychiatric nurses and four para-professional. • The average stay is between 1-4 month. • Pts and relatives found it less stigmatizing then the control service and less upsetting similar results have been for intensive cause management – perhaps a less hospital type environment should be the choice for first episodes if they are not extremely sever.

Community approaches for acutely ill patients • Santa Clara County Clustered (Apartment project) • The ( SCCCAP) was not established as an alternative to hospitalization the creators of the programme argue that to regard the pt subculture as negative phenomenon is not appropriate the model was based on the simple concept that if a pt. community was given the resources and the assistance to form itself on appositive mutually supportive basis. It could act as a permanent support system . • The model used the clustering of pt`s homes as one vehicle for fostering a community . • Community organizers' rather than clinicians would assist the clients in establishing a mutually supportive and interdependent community.

  18. Community approaches for people with enduring and sever mental illness • Gwydir Project ,Cambridge, UK • It was established as multi- agency response to the need to provide intensive health and social care in the community for high risk groups of people who have sever mental health problems and complex need. • They provide practical and emotional help aimed at maintaining the client in the community an improving their quality of life . • The criteria which were agreed for referral to the project are: • Sever mental illness. • Vulnerability with a risk of harm to self and others. • A history of non engagement or lack of co-operation with services. • A need for intensive support to prevent breakdown of existing functioning.

  19. Cont……. More men than women were referred to the project ( 17-24) They were predominantly young ( 52% Under 40 years) The referring agents indicated the need for support. Gwydir pts conform to published definition of sever mental illness. The project successfully engaged a number of clients, some of whom had difficulty engaging with statutory services. Gwydir project unit costs appeared to be similar to those of an occupational therapy visit. The project met its objectives to engage some of the most difficult mentally ill client group.

  20. Critical components… • What appear to be the critical components that lead to successful alternative treatment model? Are there aspects of alternative model that look promising and consequently suggest avenues for further research?

Selection Criteria… • A broader community treatment programme clearly do not exclude pt. who are violent or criminal charges, • ( AIDS) pt. or other medical problems. • Most of the programme mentioned.. do have such exclusion criteria so although it is possible to treat most of these pt in certain settings and under certain circumstances require more traditional inpatient care.

  22. Services Elements… • The pts they treat must either being diverted from the other conventional treatment facilities or they must be those who would never have reached treatment.

  23. Medication A critical element in many of the alternative services is the reduced dosage of medication required and the use of oral medications and monitoring.  Monitored oral medication has at least two advantage: The monitoring of side effects on basis and the adjustment of medication to reduce these. The monitoring demands face to face contact which may also be active to enhance continuity of care.

  24. Continuity Of Care Continuity of care emerges as a key variable in successful treatment The discharge planning As an integral part of the residential experience can help to ensure continuity of care after treatment .

  25. Program Content Some benefits seem to derive from the nature of the residential programs themselves: Characteristics summarized by (warner 1995) are: Retain autonomy Maintain community links Less stigma Maintain social skills More support = lower medications Improves access to care Enhances continuity of care Lower costs.

  26. Personnel Another important aspect of the success of such programs is the quality of the personal involved and their knowledge ,experience and supervision.

  27. Evidence The evidence for the success of these programs is more after asserted than demonstrated. Lack of scientific evidence for effectiveness or efficiency dose not prevent program being replicated.

  28. Conclusion Success in both aspects of care can only be achieved by studying whole service system in different cultures.

  29. بدائل العلاج النفسي في مجتمعنا الفلسطيني

  30. أولاً: العلاج بالقرآن لقد أصاب العلاج بالقرآن ما أصابه من تزييف وتخريب في شكل شعوذة ودجل واستغلال متنوع بلا خوف ولا وجل حتى نادى البعض بالإقلاع عنه وتركه إلى الوعي الفردي والعلاج الذاتي . ولقد سمعنا كثيراً عن العلاج بالقرآن واختلاطه مع بعض الأساليب الأخرى لبعض المشعوذين والفتاحين والفتاحات فهؤلاء لهم أعمال يندى لها الجبين ... فهل نلغي العلاج بالقرآن لأجل عيون الدجالين إخوان الشياطين ؟؟ لم يجبرنا الشياطين على أن نترك العلاج بالقرآن فهل يفعل ذلك الدجالون والمشعوذون؟ إن فعلنا ذلك نكون كالذي يقر ترك الإسلام .

  31. أولاً: العلاج بالقرآن نحن نجيد الملاحظة ولا نجيد التفسير وهناك حالات مشتركة بين المرض العضوي والنفسي والمس وهناك حالات غريبة تعالج بالقران بنجاح مؤكد دون تأكيد السبب والنتيجة أو العلاقة السببية بينهما . وهناك علامات للدجالين والمشعوذين والواهمين ولكن ما نؤكده أم المسلم يجب أن يكون مثقفاً واعياً مميزاً يفرق بين الإيمان والتفكير الخرافي وبين العلاج الحقيقي والشعوذة وبين المعالجين والدجالين حتى وإن تمسّحوا للتمويه بشيء من الدين . والأسلوب المتبع للعلاج بالقرآن هو ما يعرف بالرقية الشرعية.

  32. الرقيـــــة الشرعيـــــــة الرُّقْـية: العوذة، والـجمع رُقًـى، وتقول: اسْتَرْقَـيْتُه فرقَانـي رُقْـية ، فهو  راقٍ، ورجل رَقَّاءٌ: صاحبُ  رُقًـى. يقال:  رَقَـى الراقـي رُقْـيةً  و رُقـيّاً إِذا عَوَّذَ ونفث فـي عُوذَتِه ، قال ابن الأَثـير:  الرُّقْـية العُوذة التـي  يُرْقـى بها صاحبُ الآفةِ كالـحُمَّى والصَّرَع وغير ذلك من الآفات.

  33. صفــــة الرقيـــــة أن يقرأ الراقي على محل الألم ، أو على يديه للمسح بهما ، أو في ماء ونحوه ، وينفث إثر القراءة نفثا خاليا من البزاق ، وإنما هو نفس معه بلل من الريق أ.هـ "فتح الباري كتاب المرض".

  34. ثانياً: الجن وتأثيره على الإنسان تأثير خارجي وذلك من خلال الوسوسة. يؤثر من الداخل (يدخل الجسم المخ ومركز العصب). وحسب إحصائيات "أبو همام "معالج بالقرآن أن 95% من المشاكل هي حسد، اكتئاب، مشاكل عائلية واقتصادية ، و 5% حالات سحر، وتعالج بالرقية الشرعية. ويفيد المعالج "أبو همام" بأنه التقى مع الدكتور عايش سمور مدير الإدارة العامة للطب النفسي حيث أفاد الدكتور وهو صاحب خبرة طويلة في هذا المجال أن الرقية تفيد حوالي 40% في علاج حالات نفسية وهو يفكر بجدية بإدخال الرقية في برنامج العلاج النفسي.

  35. قصة واقعية:جنِّي يسبب انخفاض السكر لفتى فتى لم يتجاوز الحادية عشرة من عمره وهو من الموهوبين في مدرسته كان يشكو من انخفاض في كمية السكر حسب التقارير الطبية تؤدي إلى دخوله في غيبوبة وكان ينتابه الإغماء في مدرسته ويدخل الغيبوبة لأكثر من مرة وعند نقله إلى المستشفى يتم فحص السكر له فيكون منخفض جداً لدرجة لا تصدق مع العلم أن والده أخبرني أنه يتضايق من سماع القرآن أو الذكر أو العبادة، وينتابه القلق، وأصبح لا يحب مدرسته ويصاحبه الصداع المستمر وله ملف صحي في مستشفى من المستشفيات وكان يتابع العلاج مع أهله وعندما قرأت الرقية من القرآن وبعضاً من الأدعية المأثورة عن رسول الله صلى الله عليه وسلم نطق جني عليه ودار الحوار التالي:

  36. قلت: أقسم عليك بالله العظيم رب العرش العظيم أن تنطق على لسان هذا الفتى ولا تؤذيه بسم الله من معنا: قال: أنا جاليلو قلت : ما ديانتك . قال : نصراني قلت : ما لونك قال : أحمر قلت : كيف دخلت في بدن هذا الفتي ؟ قال : عن طريق السحر . قلت ماذا تسبب لهذا الفتي ؟ قال : أنا أقوم بمص وسحب السكر حتى ينخفض السكر في جسم الفتى . قلت : أنت تعلم أنك معتد أثيم . أليس هذا ظلماً وقد حرم الله الظلم على نفسه وجعله بين الخلق محرماً . هل آذاك هذا الفتي " يقول ربي الله " قلت مكرراً ما قاله مبينناً عدم صحة ما يردده ثم عرضت عليه الإسلام : قال موافق قلت له : قل " أشهد ألا إله إلا الله وأن محمداً رسول الله وكررتها له .....

  37. قلت : ما لونك الآن وكم من القرون لك ؟ قال لوني أبيض ولا يوجد أي قرن لي . كان ضمن الحضور أطباء فسأل سؤالاً هل أنت السبب في انخفاض السكر لهذا الفتى ؟ قال : تعم قال الطبيب هل باستطاعتك أن تثبت ذلك ؟ قال : ماذا تريد مني قال الطبيب: أنا سأقوم بقياس الضغط بدون تدخلك ثم أقيس ثانية بتدخلك يعني أن تعطيني قراءتين للضغط . قال هذا أمر سهل بالنسبة لي إذاً كنت أنا السبب في انخفاض السكر فأنت تطلب مني أمراً سهلاً. فقلت له مذكراً إياه والجالسين بالحديث الشريف " إن الشيطان يجري من ابن آدم مجرى الدم بالعروق" سأله الطبيب لكن أخبرني ماذا ستفعل أنت ؟ فقال : أنت سمعت الآن ما قال الشيخ " أليس نجري من ابن آدم مجرى الدم قالها ووجهه أحمر وغضب . فقلت له يا أخي لا تغضب أنت مسلم الآن والطبيب متخصص في القلب ويريد أن يسألك . قال الجني : أنا أعتذر ولكني فكرت أن الطبيب لم يصدقني وهنا طلبت منه بداية قياس الضغط دون تدخله قام الطبيب بقياس الضغط وكان 110/70 وبعدها طلبت من الجني التدخل فقام الطبيب بالقياس مرة أخرى فكانت النتيجة 145/110 فتدخلت وقلت على الفور مقسماً على الجني أن يترك القلب والشرايين لهذا الفتى قام الطبيب بقياس الضغط مرة أخرى رجع الضغط كما كان عليه . المصدر: *** كتاب المنهج الرباني في علاج السحر والعين والمس الشيطاني للدكتور/عبد الفتاح الهمص

  38. من خلال هذه القصة نبين الحقيقة التالية: الجني يسبب مرضاً عضوياً للإنسي ولكن ليس كل مرض سببه الجن . يمكن معالجة هذه الإمراض عن طريق خروج الجني .

  39. ثالثاً: العلاج النفسي الديني للقلق نزل القرآن الكريم أساساً لهداية الناس أجمعين, ولدعوتهم إلى عقيدة التوحيد, ولتعليمهم قيماً وأساليب في التفكير والحياة, ولإرشادهم إلى ممارسة السلوك السوي , بما فيه من صلاح الإنسان وخير المجتمع , كما أوضح القرآن الكريم لبني البشر الطرق الصحيحة لتربية النفس وتقويمها وتنشئتها تنشئة سليمة, تؤدي بها إلى بلوغ الكمال الإنساني, الذي تحقق به سعادة الإنسان في الحياة الدنيا والآخرة, قال تعالى: " إن هذا القرآن يهدي للتي هي أقوم ويبشر المؤمنين الذين يعملون الصالحات أن لهم أجراً كبيرا". ( الإسراء: 9) وقال أيضا: "بل الإنسان على نفسه بصيرة * ولو ألقى معاذيره" ( القيامة: 14-15). ولا شك أن في القرآن الكريم طاقة روحية هائلة , ذات تأثير بالغ الشأن في نفس الإنسان فهو يهز وجدانه, ويرهف أحاسيسه ومشاعره, ويصقل روحه, ويوقظ إدراكه وتفكيره, ويجلي بصيرته, فإذا بالإنسان يصبح إنساناً جديداً كأنه خلق خلقاً جديداً.

  40. ثالثاً: العلاج النفسي الديني للقلق لقد بذلت في الآونة الأخيرة جهود مضنية في ميدان العلاج النفسي, للمصابين في الاضطرابات الشخصية والأمراض النفسية, غير أنها لم تحقق المرجو منها وهو القضاء على الأمراض النفسية والوقاية منها, كما بدأت كذلك تظهر حديثاً اتجاهات جديدة في ميدان العلاج النفسي, تنادي بأهمية الدين في الصحة النفسية, وفي علاج الأمراض النفسية, وترى أن في الإيمان بالله تعالى قوة خارقة تمد الإنسان – المحافظ دينياً- بطاقة روحية هائلة, تعينه على تحمل مشاق الحياة وتجنبه القلق والوساوس التي يتعرض لها كثير من الناس الذين يعاركون الحياة المعاصرة, والتي تسيطر على جميع مظاهرها المختلفة الاهتمام الكبير بماديتها البراقة, التي تفتقر في نفس الوقت إلى الغذاء الروحي, ما سبب للإنسان المعاصر كثيراً من ألوان الضغط والتوتر النفسي, وجعله نهيباً للقلق, وعرضه للإصابة بالأمراض النفسية.

  41. رأي علماء النفس غير المسلمين نعرض فيما يلي بعضاً من أقوال أصحاب الفكر من غير المسلمين حول العلاج الديني للقلق: يقول دائيل كارنجي في كتابه " دع القلق واستمتع بالحياة": إن بين من نادى (وليم جيمس) William James فقد قال : إن أعظم علاج للقلق ولا شك هو الإيمان, وقال أيضاً: الإيمان من القوى التي لا بد من توفرها لمعاونة المرء على العيش, وفقده نذير بالعجز عن معاناة الحياة, وقال أيضاً: إن بيننا وبين الله رابطة لا تنفصل, فإذا نحن أخضعنا أنفسنا لإشرافه تعالى تحقق كل أمنياتنا وآمالنا, وقال أيضاً : إن أمواج المحيط المصطحبة المتقلبة لا تعكر قط طمأنينة التقلبات المؤقتة.

  42. رأي علماء النفس غير المسلمين قال كارل بونج C. Jung : استشارني خلال الأعوام الثلاثين الماضية أشخاص من مختلف شعوب العالم المتحضر, وعالجت مئات كثيرة من المرضى .. فلم أجد مريضاً واحداً من مرضاي الذين في المنتصف الثاني من عمرهم ممن لم تكن مشكلته في أساسها هي افتقاره إلى وجهة نظر دينية في الحياة, وأستطيع القول إن كلاً منهم قد وقع فريسة المرض, لأنه فقد ذلك الشيء الذي تمنحه الأديان القائمة في كل عصر لأتباعها, وأنه لم يتم شفاء أحد منهم حقيقة إلا بعد أن استعاد نظرته الدينية في الحياة.

  43. رأي علماء النفس غير المسلمين • يقول أ. أ . برين A.A.Brin المحلل النفسي: إن المرء المتدين حقاً لا يعاني قط مرضاً نفسياً. • ذكر هنري لينك Hnery Link العالم النفسي الأمريكي في كتابه ( العودة إلى الإيمان): أنه وجد نتيجة خبرته الطويلة في تطبيق الاختبارات النفسية على العمال في عمليتي الاختبار المهني والتوجيه المهني أن الأشخاص المتدينين والذي يترددون على دور العبادة يتمتعون بشخصية أقوى وأفضل ممن لا دين لهم أو لا يقومون بأية عبادة.

  44. رأي علماء النفس غير المسلمين كما أشار كثير من المفكرين الغربيين المحدثين إلى أن أزمة الإنسان المعاصر إنما ترجع أساساً إلى افتقار الإنسان إلى الدين والقيم الروحية, فقد أشار المؤرخ أرنولد توينبي A.Twinby إلى أن الأزمة التي يعاني منها الأوروبيون في العصر الحديث إنما ترجع في أساسها إلى الفقر الروحي, وان العلاج الوحيد لهذا التمزق الذي يعانون منه هو الرجوع إلى الدين.
