Terminal Learning Objective (TLO) • Action-Teach Soldiers how to properly complete a DD Form 93 (Record of Emergency Data) and demonstrate what a completed form should look like • Condition: In a classroom environment given a blank DD Form 93, students personal notes and a scenario • Standard: Soldiers will be able to complete a DD Form 93 with zero deficiencies

  5. Risk Assessment Level • Low

  6. Safety Requirements • Be careful moving around the classroom as there are some loose wires. • Ensure all drinks have lids to prevent electrical shock

  7. Administrative Notes • Latrine is located down the hall on the left. • Designated smoking area is out the back door.

  8. Environmental Considerations It is the responsibility of all Soldiers and DA civilians to protect the environment. Please recycle.

  9. Evaluation • Students will be evaluated by a Check on Learning at the end of this block of instruction. This grade will be a GO/NO-GO. • Students will also assist the instructor in the demonstration. • Students will be given a practical exercise.

His SSN is 123-45-6789. His UIC is W92MAA in Eastover, SC. Jessie Hunt lives at 123 Happy Lane, Anytown, SC 29044. Jessie’s phone number is (555) 555-5555. SSG Hunt has a son, Anthony L. Hunt who resides with his ex-wife, Cheryl Hunt at 125 Sad Lane, Othertown, SC 29045. Anthony’s date of birth is July 1, 2000. Cheryl’s phone number is (444)444-4444. Bobby and Jessie have a daughter, Hailey B. Hunt that lives with them. Hailey’s date of birth is April 15, 2007. Bobby does not know who his father is. Bobby’s mother’s name is Naomi Hunt but she died a few years ago. Bobby wants to ensure that if he is in a missing status that his friend Will Burdette is informed of his status. Will lives at 67 Turtle Lane, Eastover, SC 29033 and his phone number is (333)333-3333. Bobby wants his death gratuity to go to his wife Jessie. Bobby wants Jessie to be his PADD and he wants to leave all of his unpaid allowances to her. Bobby wants his Non Medical Attendant to be Jessie. Bobby’s preferred geographic location is Columbia, SC.

  35. Practical Exercise Scenario • Today is June 4, 2013 and PFC Robert L. Camelbak who is in your unit comes to you asking for help updating his DD 93. PFC Camelbak is single. His SSN is 987-65-4321. His UIC is W8FCA3 in Greer, SC. PFC Camelbak has no children. PFC Hunt’s mother and father are still married and live at the same address with the same phone number. His mother’s name is Betty Boop and his father’s name is Bubba Boop. They live at 78 Lipton Lane in Patrol, TX 44345 and their phone number is (803) 867-5309. PFC Camelbak wants his death gratuity to go to his father. He wants his mother to be his PADD, NMAD and he wants to leave all of his unpaid allowances to her. PFC Camelbak’s preferred geographic location is Patrol, TX. Fill out the DD Form 93 appropriately.
