  2. 1 Patient sees GP Dan presents to his GP, Dr Sarah Carson, with a painful left knee. His wife is with him to help interpret as he doesn’t speak English very well. Diagnostic Referral

  3. 2 GP initiates diagnostic referral Following examination, Dr Carson suggests a knee x-ray and logs onto the NHS e-Referral Service, typing “knee x-ray” as the reason for referral. Diagnostic Referral

  4. 3 GP completes Clinical Request Template (CRT) Dr Carson completes the required Clinical Request Template (CRT) and answers questions confirming that Dan is suitable for the diagnostic test. Once the CRT is submitted, the NHS e-Referral Service shows providers of knee x-rays. Diagnostic Referral

  5. 4 Patient chooses diagnostic provider Dan would like to be seen as soon as possible so is happy to travel further afield if necessary. Dr Carson looks at all the options available (NHS and independent providers) and finds an appointment available the following day, which they book. Diagnostic Referral

  6. 5 GP receives results Dan attends the appointment and Dr Carson, having previously chosen to receive the results electronically via the NHS e-Referral Service, is notified that they are available via her worklist. The results indicate moderate osteoarthritis. Diagnostic Referral
