  3. Registration - How to Register for a new online account • Go to www.perkinelmer.com • Click on Shop Online • Click on Register • Fill out the form on the next page. You MUST include your account number otherwise you cannot order through UNC. • For Radioactive products use account # 100539566 • If you don’t know your account number, please contact your sales rep, Julie Ginsler 919-522-6741 Page 3

  4. Registration - How to Register for a new online account • On this screen, select the department number that corresponds to your departmental number. • This number is used to help PerkinElmer and UNC completed the UNC Monthly summary billing Page 4

  5. Registration - How to Register for a new online account • Confirm your Shipping and Billing Address here (only one ship to address for radiation 1120 Estes Dr Ext) • Once you complete this step, you are ready to order online. Page 5

  9. Ordering – How do I place an order? • Once you find the item you need, simply click “Add to cart” • Also note, you can see your customer-specific pricing here! Page 9

  10. Ordering – How do I place an order? • Once you have placed all of your items into the cart, confirm shipping dates, pricing, quantities then click on “Proceed” Page 10

Ordering – How do I place an order? • Confirm the shipping address is correct, add in your attention line and any special instructions that you would like Customer Care to review. • Add building and room number for Environment, Health & Safety personnel (internal delivery instructions) • Once completed, click on “Proceed” Page 11

Ordering – How do I place an order? • Confirm your billing address and add in any promotional codes you may have for the order. • IMPORTANT: Enter your full 10-digit FRS grant number in the Purchase Order Field. K #’s (Check Requests), Credit Cards and W #’s will not be processed correctly. Please use only your FRS Grant number so that the invoicing process works correctly and your order will be delivered without delay by EH&S. • Click “Proceed” Page 12
