Confirmation of Registration

1. CONGRESS REGISTRATION FORM *Participants having applied for new DKGH membership and paid their contribution before August 15, 2008 may pay the Member‘s fee. Membership information is available atastridmilic@hotmail.com Renewals of existing memberships can be made on site. ** Please provide a copy of a Student‘s ID or a confirmation signed by the Head of Department at the moment of your registration by fax to +385 1 45 96 942 or together with the hardcopy of the registration form. Confirmations handed in at a later stage cannot be considered. Confirmation of Registration You will receive written confirmation of registration by mail when the registration form has been received and the payment has been credited to the congress account. If your registration form is received between October 15 and November 15, 2008, you will be pre-registered for the conference, but no confirmation of payment will be sent. If you make a bank transfer, please bring proof of payment of your registration fee, when collecting the registration material. After November 15, 2008, pre-registration will be closed, but you may register at the "On-site registration Desk" in Croatian Institute for Brain Research in Zagreb, which will open on FridayNovember28, 2008, at 14.00 hrs. Please do not perform a bank transfer after November 15. Please note: Pre-registration without payment of the registration fee is possible. The (non-reduced) fee will then have to be paid at the registration desk (cash in EUR or Kn). Cancellations and Refunds Before November 27, 2008: 90% refund
