KOHP Summit - 01-25-11 • 48 participants from Sedgwick, Harvey and Butler Counties and State agencies • Convened by KDHE & KDOA • Purpose: to join others in your community to shape the vision for the Self‐Management Program. Resulted in Sedgwick County participants wishing to continue meeting on a monthly basis - this was the start of the Sedgwick County KOHP Board

Sedgwick County KOHP Board Members • Kansas Department of Health and Environment • Central Plains Area Agency on Aging • The University of Kansas School of Medicine – Wichita • Sedgwick County K-State Research and Extension Office - Adult Development and Aging • Wichita State University • Health Strategies • The Arthritis Foundation • Wesley Medical Center • The Regent Senior Living • Bel Aire Senior Center

Development of Sedgwick County KOHP Board • February 25, 2011 - First meeting • Discussion focused on results of the Summit – addressing logistical barriers and challenges to growing KOHP in Sedgwick County • WSU Aging Studies and the Sedgwick County K-State Research and Extension Office offered leadership in addressing logistical challenges • WSU Aging Studies would provide student help • Sedgwick County K-State Research and Extension would serve as the central administrative site and supervise students • Kansas Health Foundation Recognition Grant application

KOHP Barriers/Challenges:Time and Administrative Burden • Identifying and making arrangements for course locations • Development of charts for use in course • Healthy snacks • Identifying co-leaders and alternates • Lack of central location for course registration • Lack of consistent course cost

KHF Recognition Grant • Purpose: to develop a systematic delivery of KOHP to provide consistent availability of the program in Sedgwick County • Centralized program administration • Support for the program facilitator teams • Reduction in time spent by facilitators in administrative tasks • Creation of an efficient, stream-lined system • Consistent provision of KOHP workshops • Development of a participant scholarship fund

  8. KHF Recognition Grant • Develop: • Workshop supply kit (charts, markers, name plates, etc) • System for managing logistical arrangements • System to provide participant snacks • Develop marketing and outreach plan • Create supports to meet course leaders’ needs on an on-going basis

KHF Recognition Grant • Develop community partners across the county who are willing to host the program in their facilities • Develop a system for referring program participants through community partnerships • Coordinate the scheduling of the program and participant referrals and registrations through a centralized system housed within the Sedgwick County K-State Research and Extension Office and supported by Aging Studies graduate students.

March 31, 2011 Board Meeting • Development of Wiki for communications purposes • Identification of centralized payment location and agreement on consistent pricing of course • Discussion of scholarship criteria • Selection of consistent meeting time and place • Development of bookmark/pamphlet to be distributed at area doctor offices and other potential referral sites
