Full Service Community Schools grant - Performance Measures Close the Achievement Gap • Increase the percentage of FSCS students that meet or exceed Illinois standardized achievement tests (PARCC) for math and reading by 12% each year (to obtain a 60% increase over 5 years) as evidenced by the number of students participating in the program that meet or exceed Illinois standardized achievement tests. • Increase the percentage of FSCS students that meet or exceed their NWEA Measures of Academic Progress growth targets for math and reading by 10% each year (to obtain a 50% increase over 5 years) as measured by Northwest Evaluation Association (NWEA) Measures of Academic Progress (MAP) scores. • Increase the percentage of preschool students participating in the program that enter kindergarten performing math and language/literacy skills at grade level by 16% (to obtain an 80% increase over 5 years) as measured by the Desired Results Development Profile (DRDP) for students that will be entering kindergarten the following year. • Increase student school day attendance by 2% per year (to obtain a 6% increase over 3 years) as measured by District attendance reports.

Full Service Community Schools grant - Performance Measures Cont’d Improve Student and Family Health • Increase the percentage of clients that received mental health services that completed at least one treatment goal by 16% each year (to obtain an 80% increase over 5 years) as evidenced by reports. • Increase the number of individuals (students, family members, and community members) that participate in a healthy lifestyle opportunity by 20% each year (to obtain a 60% increase over 3 years) as evidenced by sign-in reports. Enhance Parent Stability and Engagement in their Child’s Life • Increase the percentage of parents participating in parenting education opportunities by 20% each year (to obtain a 100% increase over 5 years) as evidenced by program attendance records. • Increase the percentage of adults participating in English as a Second Language (ESL) classes that demonstrate a minimum gain of one or more performance levels by 13% (to obtain a 65% increase over 5 years) as evidenced by Literacy DuPage and College of DuPage assessments. • Increase the number of individuals who receive linkage and access supports by 10% each year (to obtain a 30% increase over 3 years) as evidenced by daily staff reporting logs.

Embedding Qualitative Data From a FSCS Executive Summary: Access and linkage to community resources in order to remove barriers to academic success is one of our most important supports available to families. Recently, a Family Liaison was working with a family with three children, all of whom were late to school every day at the start of the school year. The Family Liaison connected with this family, learning about the barriers that were preventing them from making it to school on time. She developed strategies with the mom to help encourage her children to wake up on time and be ready for the bus. From these strategies, the oldest child took it upon themself to be responsible for waking up their siblings in order for them to be at the bus stop on time. After four months of working with the Family Liaison, all three children have nearly perfect attendance and arrive to school on time daily. This family is one of 2,489 that was supported by a Family Liaison during this reporting period.
