Introduction The patient has difficulties in passing urine. It is caused by many pathogenic factors. In TCM, it is considered as Long Bi. Main symptoms The patient has abdominal distention owing to the retention of urine since he cannot pass water. A history of operation, traumatic injury and windstroke is present.
Etiology and pathogenesis Its root cause is kidney qi deficiency, failure in controlling opening and closing of bladder. • The discharge of urine depends on the qi activity of bladder which relies on the warming of kidney. When kidney qi is deficient, the qi activity of bladder is not able to make the urine discharged. • Qi stagnation, blood stasis and damp heat left by operation, traumatic injury or windstroke are also the inducing factors of retention of urine.
Example case Liu x x, female, 49 years old, had retention of urine for 20 days after operation for hemorrhoids. She had desire to pass urine but could not urinate. • Her tongue is slightly red with white sticky coating and wiry rapid pulse. Treated by Chinese and western medicine, she was not relieved. So she came for acupuncture treatment.
Treatment Shenshu (BL 23), Pangguangshu (BL 28), Qihai (CV 6),Guanyuan (CV 4), Zhongji (CV 3), Shuidao (ST 28) and Yinlingquan (SP 9) were selected. The abdominal points were punctured obliquely downward to prevent penetrating the bladder. • After the needling sensation came, a moxa stick was used to do moxibustion for 30 minutes. After removing the needles, hot compress was done at the lower abdomen. • On the 3rd, the catheter was removed because she could pass urine. Five days later she was cured.
