Case management Higher Education Case Managers serve their University and individual students by coordinating prevention, intervention, and support efforts across campus and community systems to assist at risk students and students facing crises, life traumas, and other barriers that impede success. -http://www.hecma.org/mission--vision.html

Off-Campus referrals • Counseling, psychiatry, psychological evaluation, groups, etc • Bridging process • Information needed: • Insurance or Self-Pay? • If insurance give company name and type (e.g. Blue Cross Blue Shield HMO, Aetna PPO Choice Plus, etc.) • If Self pay, what is the maximum amount they can pay out of pocket per week for counselor or psychiatrist? • Issue of concern that referral will address (e.g. depression, eating disorder, etc.) • Do they have a car or a transportation option? • What is the zip code of where they live? How far are they willing to drive?

  3. Higher level of care • Inpatient • Not Treatment; Stabilization • Residential treatment • 30 to 120 days • Partial hospitalization programs • 5 hours per day for 3 to 5 days per week • Intensive outpatient programs • 3 hours per day for 3 to 5 days per week

  4. Hospital discharges • Assessments • In depth review of prior treatment history • Procedures • Multiple ROI’s (avg 5 to 8) • Medical emergency letter • Coordination with professors • 3 appointments within one week • Community Treatment attendance verification process

  5. Case management • Academy for inclusive learning • Autism spectrum disorder support • Adhd/psychological testing referral • KSU Student Disability Services accommodations • Coordination of care with multiple providers • Heal team • High risk/students frequently in crisis/Distress

  6. Medical case management • Patient assistance programs • Sample medication • Medication tracking and Audit • Psychiatric Record requests and fulfillment

  7. CARE The Campus Awareness, Resource & Empowerment (CARE) Center offers support to students who have experienced homelessness, food insecurity and/or the foster care system.

  11. TO refer or not to refer?? • A student mentions to you they have decided that they need to look for a part-time job.

  12. TO refer or not to refer?? • A student has rheumatoid arthritis and tells you that they are experiencing flare ups because they lost their health insurance and are no longer able to afford their doctor visits or medication

  13. TO refer or not to refer?? • During an Initial Consultation, a student reports multiple mental health hospitalizations within the past 12 to 18 months. They currently have an off-campus psychiatrist and do not wish to transfer services to CPS at this time.

  14. TO refer or not to refer?? • A student tells you that they failed the majority of their classes last semester and are planning to complete an Academic Standing Appeal and ask you to write a letter of support for them.

  15. TO refer or not to refer?? • A student tells you that they have nowhere to live and are currently staying at a friends apartment. When you ask why they can’t stay with their family, they say that their house is too far away and they don’t want to commute.
