  1. 柚子汁對Sildenafil在大白鼠CYP3A4的代謝影響及體外藥物交互作用動力學模式之建構柚子汁對Sildenafil在大白鼠CYP3A4的代謝影響及體外藥物交互作用動力學模式之建構 • Sildenafil對於磷酸二酯酵素第五型具有專一性的抑制效果,此作用會使得陰莖內血管持續擴張、陰莖海綿體肌肉鬆弛,達到勃起效果。Sildenafil經過人體吸收後最主要是經過肝臟酵素CYP450 3A4代謝成同樣具有藥理活性的代謝物,根據本實驗室之前的研究顯示,Sildenafil與葡萄柚汁併服時,葡萄柚汁會抑制Sildenafil的代謝使得血漿中的Sildenafil濃度增加,代表Sildenafil在與葡萄柚汁併服後Sildenafil被CYP3A4代謝的酵素動力學改變。除此之外也有學者證實,CYP3A4具有多重選擇性的代謝模式。本研究中會以Michaelis - Menten kinetic model評估Sildenafil與葡萄柚汁以及柚子汁在microsome的酵素動力學模式,並且利用二個已知的auto-activator藥物 (Testosterone和Carbamazepine) 評估Sildenafil在CYP3A4活性結合區的代謝特性。實驗結果顯示,Sildenafil在microsome的酵素動力學是遵循Michaelis - Menten kinetic model,其Vmax = 1.96 μMmin-1,Km = 27.31 μM. 。另外Sildenafil與葡萄柚汁以及柚子汁並用時,在microsome的酵素動力學也是遵循Michaelis - Menten kinetic model,其Vmax 沒有明顯的改變,Km 則有增加的趨勢。進一步以Lineweaver-Burk plot評估,結果顯示,葡萄柚汁以及柚子汁對於CYP3A4的抑制作用模式是屬於競爭性抑制模式。利用Testosterone和Carbamazepine評估Sildenafil在CYP3A4的活性結合區的實驗,結果顯示Sildenafi在葡萄柚及柚子存在下,經由CYP3A4進行N — demethylation代謝作用與Testosterone和Carbamazepine進行代謝作用是屬於互相抑制的現象,彼此之間會相互影響,結果也顯示,Sildenafil與Testosterone和Carbamazepine,在CYP3A4的鍵結牽涉到一個以上活性位置的動力學模式。
