Falls Rate per 1000 Clients (Outcome Measure) 2. Percentage of Falls Causing Injury (Outcome Measure) 3. Completed Fall Risk Assessment on Admission (Process Measure) 4. Forty-sevenImprovement Teams from Long Term Care, Acute Care and Home Health Care facilities across the country enrolled in the Falls Virtual Learning Collaborative, and made a one year commitment to implementing best practices (Getting Started Kit) and conducting tests of change (Change Package) to reduce falls and injury from falls by 40% by March 2011. Team used virtual technology to meet for four learning sessions.
