  3. McKinney Vento • Example (review team mentioned this specific example numerous times) • Mom is currently not working, she is living with her boyfriend he provides housing and pays the bills.   4. McKinney Vento • Mom brought in birth certificate, shot record form 2050 and BCBS card, and referral letter from TTUEHS. Mom said she and her children live with her friend because she and husband separated last month. • Mom came in to do application. She brought birth certificate, shot record, and Medicaid card. Family was referred by Ballenger Head Start. Mom and dad do not work and they live with (dad) grandparents. Parents have not worked in over a year. Mom said they do receive Food stamps, Medicaid and WIC. Homeless Inconclusive

  5. McKinney Vento • Examples • Mom provided shot record and birth certificate. Mom stated that her and her four children are living with her mom and that she is pregnant with her fifth. She is looking for a job at this time, but is going to wait until the baby is born in September. Mom stated that the children are on Medicaid but that she didn't have the new card with her. • This is a single parent family. Mom is unemployed and is going to start College August 2010. Family is living with Grandparents. Family is not under section 8 housing. Family is receiving food stamps. Child is on WIC and Medicaid. Family is not receiving CCS or TANF. Inconclusive Inconclusive

McKinney Vento • parent came in to complete application. she stated that she is currently living with a friend and seeking employment. she is a single parent with 4 children. she brought in copy of Medicaid, shot record , b-certificate. • Mom is living with her sister due to economic needs. She is a single mom with one child. She used to received TANF but not anymore, she is receiving food stamps and child has Medicaid. Brought Birth Certificate, Shot Record and Medicaid Inconclusive Homeless

Mom does not work and has NO income at this time. She is currently looking for a job. Family is in transition due to separation and divorce. Inconclusive Inconclusive Inconclusive

No Income Declaration • Must have’s: • Printed Name & Signature • Date • No income for last 12 months • What Ifs • Parent worked 1 week in last 12 months? • Must provide pay stub for that 1 week, then would not need declaration • Parent worked 9 months in last 12 months? • Must provide pay stubs for those months, then would not need declaration • Parent worked last year? • As long as work was not done in the last 12 months from recruitment date, the parent does not need to provide income, but must sign an income declaration
