  2. The Multiple Award Winning BGreen £20million improvement programme covering 1400 homes in St Mary’s and Coldhurst led by First Choice Homes. Great example of whole system-whole place.

Early Help delivers results • 4237 households; 7595 unique individuals supported. • 92% of families maintained engagement • 85% of households show an improvement in their scores between the initial and final assessment. • Over 65% of people reported an increase of physical activity at three month follow-up. • Over 50% of people who have accessed the service with the intention of losing weight have achieved a 5% weight loss by case closure.

Your Oldham • Inform residents about and promote the good initiatives taking place in Oldham, showing how people and organisations are doing ‘their bit’ • Engage residents in a different conversation which puts them and their skills, passions and abilities at the centre of the discussion and • Inspire residents to do ‘their bit’ and get involved Since launching the Your Oldham in September 2016, we have held co-operative conversations with over 500 people, gathering a broad spectrum of ideas, stories and feedback which are helping shape the Your Oldham Festival. We have also recruited 77 celebration makers to date and released the first Your Oldham film focused on food and growing – reaching over 5000 people and gaining over 1000 independent views.

  5. The Community Safety and Cohesion Partnership continues to innovate MASH goes from strength to strength and evidence of demand reduction on police call outs (relating to Domestic Violence). MP and peer review praises the Oldham approach to tackling Child Sexual Exploitation. New and ambitious CSCP plan moves beyond the traditional crime remit to tackle difficult issues and has prevention at the heart

  6. Another Award for Warm Homes OldhamGuardian Public Service Winner 2016 In its first three years helps 3,500 people out of fuel poverty; hospital visits and demand for mental health services have, in turn, been reduced.

Focussed Care: Hope Citadel • If staff think that life is just too tough and unmanageable for a patient they go onto the Focused Care programme. • One of the Focused Care Practitioners will come and visit and find out what’s really going on and from this basis, a joint plan is made involving the patient, other agencies and the practice.

All About Glodwick… “Our ambition is to become the centre of excellence for BME population healthcare” Glodwick CCG Cluster Several schemes and innovations, funded by Oldham CCG, that target the population specifically and address real health needs cutting across communities and families. These include a type 2 diabetes programme, a first language mental health counselling service, an exercise programme and a local access to equipment scheme. During the first year we have seen some dramatic changes in patient health, demonstrated through clinical results and monitoring questionnaires. The community are seeing a different side to healthcare, where the clinicians are working with them to deliver things on a local level with quick access and a defined cultural/language focus.

  18. Growing Oldham Feeding Ambition Oldham Food Network is an ambitious community-led network run by local people, for local people. Members are passionate about food, giving their time to growing, cooking, educating, sharing, supplying and eating. Through the heroic efforts, tireless dedication, commitment and support from all those involved, the crowdfunder smashed their target – twice – raising over £23,000, almost double the initial target
