  13. James starts refusing his medication and his symptoms worsen, which results in his family contacting the County Behavioral Health Department’s (CBHD) crisis center. • James is referred to the State Hospital (SH) for more intensive inpatient treatment, and a referral is sent electronically. After five days in the SH, James is discharged and his Discharge Form is sent electronically to the CBHD. • CBHD receives James’s Discharge Information. A clinician visits James in his home and during the visit, the clinician notes that James is exhibiting signs of excessive drinking, and refers him via electronic referral to another Community Mental Health Center (CMHC) provider for Substance Abuse services. • When James arrives at the CMHC, the admissions staff reviews the Referral form. During James’s session, a potential pattern of risky sexual behavior is identified, so the clinician completes a referral form and sends it electronically to the Public Health Department (PHD) for an STD screening. • James had been a past patient of the PHD. Upon receiving the referral, an STD clinic worker contacts James and sets an appointment. James has his tests ordered and the nurse sets up an appointment for James to review the results. • After returning home, James logs on to the Web portals for the different agencies to view his care team, check and record his medication, and view/update his appointments. The Connected Care Vision • After Connected Care • Electronic transfer of data for accuracy • Continuity of care • Consumer empowerment • Detailed medication history across providers • Before Connected Care • Re-keying of data, which causes errors • No continuity of care across providers • Little involvement by the consumer • No medication history
