Regulation For Medical Records • AR 40-66, Medical Record Administration and Healthcare Documentation, is the regulation that governs medical records
Purpose • provide a complete, concise medical and dental history of everyone in the Active Army or in the RC as well as Family members and other beneficiaries • used for patient care, support, and research and education • helps medical officers advise commanders on retaining and using their personnel • helps physical evaluation boards (PEBs) appraise the physical fitness of Army members and eligibility for benefits • assists in the mobilization process • dental portion (panogram) can be used to assist in identifying deceased persons
Record Organization • Records will be stored at one location and organized by terminal digit • Records are color coded by terminal digit • 0-orange • 1-green • 2-yellow • 3-gray • 4-tan • 5-blue • 6-white • 7-brown • 8-pink • 9-red • Refer to AR 40-66, fig 5-2, for left and right side documentation order
Retrieving Documentation • If SM is seen at a MTF, documentation will need to be printed from the Armed Forces Health Longitudinal Technology Application (AHLTA) • SMs being seen by a civilian provider will need to complete an Authorization for Disclosure of Medical or Dental Information (DD Form 2870) • DD Form 2870 will be sent by Care Coordinator • ALL documentation will be scanned in HRR before being filed in the STR
Transferring Records • SM will never “hand carry” record • If a SM Inter State Transfers (ISTs), the record will be sent by the record custodian to the gaining state’s central record location • Retiree records will be sent to HRC, St Louis • All records will be accompanied with a Transmittal Record (DA Form 200)
HIPAA • All Care Coordinators will act within the confines of HIPAA • HIPAA training can be completed at: http://www.tricare.mil/tma/privacy/MilitaryHealthSystemLearningPortal.aspx • Medical information is based on a need to know principle • Medical Records are property of the US Government, not the SM
