Generally… Reduce Infant Mortality Provide comprehensive care before, during, after pregnancy Provide preventive and primary care for children and adolescents Reduce teen pregnancy Reduce injury and violence Immunize all children Promote Standards & Guidelines Assess needs Coordinate w/Medicaid, WIC, etc. Specifically for CYSHCN… Provide and ensure access to comprehensive health care, including long-term care services, for CYSHCN Facilitate the development of family-centered, community-based, and culturally competent comprehensive care for CSHCN and their families. Title V Block Grant - Major Goals

Title V MCH Services and Funding Pyramid Population-Based Services Examples: Newborn and Lead Screening, Immunization, Sudden Infant Death Syndrome Counseling, Oral Health, Injury Prevention, Nutrition and Outreach / Public Education Direct Health Care Services (Gap Filling) Examples: Basic Health Services and Health Services for CYSHCN Enabling Services Examples: Transportation, Translation, Outreach, Respite Care, Health Education, Family Support Services, Purchase of Health Insurance, Case Management, Coordination with Medicaid, WIC and Education Infrastructure Building Services Examples: Needs Assessment, Evaluation, Planning, Policy Development, Co-ordination, Quality Assurance, Standards Development, Monitoring, Training, Applied Research, Systems of Care and Information Systems

CYSHCN Highlights from 2010 Report/2012 Application • OGCSHCN restructured and fully staffed • New programmatic priorities have been determined and disseminated • OGCSHCN is now ready to recruit members and hold the first quarterly meeting of a Family/Youth Advisory Council • MCHB Funding and Services Pyramid reviewed and OGCSHCN-administered and sponsored programs were re-categorized according to MCH definitions
