Purdue Racquetball Cup 2009 Registration Form - Oct 24

Join us for the Purdue Racquetball Cup on October 24, 2009! Open to all players, this tournament offers singles and doubles matches in Olympic format. Guaranteed three singles matches with lunch provided! Awards will be given for top finishers in singles and doubles events. Registration is $15 for Purdue Racquetball Club members and $35 for non-members. Entries must be received by October 21. Register via email at kgremley@purdue.edu or phone (847) 899-1337. Singles will be started in the morning following by doubles. Tournament Director: Kelly Gremley You can sign up via email (preferred method) at kgremley@purdue.edu , calling (847) 899-1337, or sending this form to: 415 Trace 4 West Lafayette, IN 47906 Entries must be received by October 21st. • Open to all players!! • You are guaranteed 3 singles matches. • Lunch provided • Awards for singles • 1stLarge mug • 2ndMedium mug • 3rd Shot glass • 1st and 2nd only for Doubles • Possible Collegiate Team event (based on top 6 players from your school) minimum of 3 players. • You will be required to sign a waiver before playing. • Must have eye protection. • May only enter on doubles event. • This is not a USAR/INSRA sanctioned event. • All skill levels welcome!
