  3. HealthCheck is West Virginia’s Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Program for Medicaid-eligible West Virginia children from birth to age 21. HealthCheck promotes comprehensive screening and primary healthcare services. HealthCheck

Medical Home • HealthCheck supports the following core values of a medical home: • Having a personal physician or primary care provider (PCP) who provides first contact care, • Ongoing care over time, • Comprehensiveness of care, and • Coordination of care across a person’s conditions, providers and settings.

  5. HealthCheck Screening Process • Initial Screen – The first check-up, either at birth or when the child enters the HealthCheck Program. • Periodic Screen – A regular check-up that happens according to a schedule. • Interperiodic Visit – A provider appointment that can happen at any time because of illness or a change in condition.

New Requirements The West Virginia Department of Health and Human Resources Bureau for Public Health has added several adolescent immunizations to the school attendance immunization requirements. Beginning with the 2012-2013 school year, all West Virginia students entering 7th grade must have had one dose of the Tdap (Tetanus, Diphtheria, and Pertussis) and one dose of the meningococcal vaccine. Students entering 12th grade must also have had at least one dose of the Tdap vaccine and a second dose of the meningococcal vaccine. If the first dose of the meningococcal vaccine was received after age 16, then only one dose of this vaccine is required. Background Highmark Blue Cross Blue Shield provided monies to WVDE for this project The health records data system is not utilized to its full potential for various reasons including but not limited to data entry burden (staffing, time and access). To address this barrier, WVDE developed a health record App to sync data (BMI, immunizations, & health screening) directly into the WVEIS system. Each county school system was provided funds to purchase a hand-held computing device (iTouch, iPhone, or iPad) to utilize this App.
