The Epping CMHT pilot study aimed to reduce hospital admissions from residential and nursing homes. By implementing a structured response team and building on existing relationships with GPs and care homes, we established a holistic approach to client assessments and behavioral management. The initiative showed remarkable results, achieving 88% admission avoidance through timely interventions and education. Positive feedback from homes and GPs underscored the effectiveness of early intervention and improved support strategies. Our findings highlight the importance of quick referrals and proactive outreach in dementia care.
