Step #1Temporary Employee Checklist and Cover Sheet • Fill-out: • Department : List the dept. he/she will be working at • Employee Name • Submitted by: usually the name of the originator • Make sure to check off every document listed once completed • Additional Notes: usually used to denote that TEMP. ALREADY HAS SOME DOCUMENTS ON FILE • Do not SAVE any documents due to modifications.

Step #2301 Hourly Temporary Employment Form • Fill-out: • Name: • Permanent Address, City, State, Zip • Department • Supervisor’s Name AND Position # • Working Title and Description of Duties • Answer YES/NO to Questions 1-3 • Current FTE • Anticipated Start Date • Appt. End Date • Position # • Hourly Rate • Fund Type: Select: STATE or FOUNDATIONEnter SIX DIGIT FUND # and ACCT. #Provide estimate of AMOUNT • Route for signatures Do not SAVE any documents due to modifications.

  5. Step #3301 Hourly Temporary Employee Contract • Fill-out: • Name • Effective Date • Working Title • Rate Type • Department • Rate per hour or term • Employee’s signature • Hiring Supervisor’s signature Do not SAVE any documents due to modifications.

  8. 301 REVISION

  9. 301 • When revisions are made the form has to be completely filled out: • The revision box checked • Stating the reason why in the working title and description of duties box • Rerouted for ALL signatures or only the signatures of the Originator and HR Personnel

Making Revisions on a 301 • Be as specific as possible when stating the reason for revision. • Acceptable forms of revision should include either ALL signatures OR only the signatures of the Originator and Human Resources Personnel • When revisions are established please notify: • HR – Maggy Sivansay • Budget – Jackie Riffle • Payroll – Debra Gunter Please include the word “REVISION” in the subject line of your email when sending notifications James Lucas Jackie Riffle David English
