  2. DS1500 • ‘Doctors Report for Disability Living Allowance, Attendance Allowance and Support Allowance or Incapacity Benefit under special rules’ • For people who are terminally ill • Suffering from a progressive disease • Not expected to live longer than 6 months

  3. Only give if Pt or their representative asks you to, the DS1500 is not a claim but a report in support of a claim • Claim will be assessed by ‘Decision Makers’ who are advised by an approved doctor. • Include only factual information, not guesses about prognosis

  4. What do the Patients get? • Claims are then treated as Urgent • Can get DLA or AA even if they don’t need help looking after themselves • Usually get IB after 52wks of sickness, with DS1500 can get IB after 28wks of sickness • Immediately given highest rate of benefit without need for medical assessment

  5. What Info is needed? • Pts name, DOB, Address • Part 1 – Condition • What is the diagnosis? • Date of Diagnosis • Other relevant diagnoses • Is the Pt aware of their condition Yes/No • Who asked you to complete form Pt/Rep (If rep needs their address) • Part 2 – Clinical Features • Part 3 – Treatment • Relevant past or current Treatment (if Palliative say so) • Planned treatments which may alter progression • Declaration

  6. Fee • Hand white copy to Pt • Fill in back of pink copy with practices bank account info so fee can be processed • NB// Once supplies of Pink and White run out different sheets will be filled in but same information
