Safer Healthcare NOW • 2006 FH signed on to all 6 initiatives of the national Safer Healthcare Now! campaign: • Rapid Response Teams • Preventing Surgical Site Infection • Preventing catheter related bloodstream infection (Central Line Infection) • AMI Acute Myocardial Infarcation Care • Ventilator Associated Pneumonia (VAP) • Preventing Adverse Drug Events - Medication Reconciliation.

  4. Group Work to reduce UTI • Ideas from the literature and other sites that had success (University of Virginia contact via NSQIP). • Insert antimicrobial (silver) catheters if patient will need a catheter for more than 48 hours.

Group Work to reduce UTI • Practice changes, pathways, standing orders to change routines (foley not inserted or removed in 24 hours) • Improve technique for insertions, use 2 people for insertion; reduce risk of contamination due to poor exposure. • Maintenance practice changes, reduce stasis (no loops), change bag q24hrs

Group Work to reduce UTI • Make removal of the foley catheter a priority; POD2 @ 0600 foley removed unless contraindicated. • Slogan: 2 DAYS TOO LONG • If catheter remains in place document the reason on the care plan. • Encourage activities to promote voiding, mobility, hydration, bowel care, relaxation.
