  4. Mrs X - Today • Medication from doctors: • GP • Hospitalizations 3 departments • Emergency ward • Private specialist • Each have their own EHR – implicate old medication info

  5. Home care service – Today • Administrate medicine to a lot of their users • Have a lack of information about their users medication and health • Typical information from doctors are via prescriptions

  6. Home care service - Today • Spends a lot of the time in telephone: • Renew prescriptions ( GP ) • Calls the hospital when a patient is discharged • Frustrating work – doctors hard to get… • Spends several hours pr week in telephone – waiting…
