  2. TWO TYPES OF EVACUATION • Rapid - must evacuate immediately • Delayed - hours to days for evacuation

  3. ACTIVATION CRITERIA • Who can make the decision to activate ? • Define how the plan is activated • Provide examples of reasons for activation • Phases of implementation (equip, notification , essential resident supplies, etc)

  4. ALTERNATIVE SITE • What sites have been identified ? • Have you negotiated with the receiving facility ? • Written MOU or similar • How do you ensure the facilities remain available? • What is the process to notify the site ? • How do you get there ?

  5. RESOURCES FOR EVACUATION • What resources / equipment is available ? • How will you move residents ? (elevators down) • Where is the equipment stored ? • How does staff access 24 / 7 ? • How is staff trained on equipment & plan ? • What is the inventory protocol ? • Are residents identified who need equipment ? • How is information kept current ?

EXTERNAL TRANSPORTATION RESOURCES • Necessary to notify EM ? • What transportation resources are secured ? • Does anyone else have them written in their plan ? • Written commitment • How are agreements kept current ? • Transportation adequate for resident needs ? • Where is this information kept & is it current ?

  7. EVACUATION DESTINATION • Do residents have a predetermined destination ? • What is the protocol to determine destination ? • Where is this information maintained ? • How is this information kept current ?

  8. RESIDENT TRACKING • What process is used to track departure ? • What process is used to track arrival ? • Who is responsible for tracking process ? • How are the residents notified and by who ? • Family / Friend emergency contact made ? • What process is in place to assure a well-organized return to the original facility ?

  9. FAMILY NOTIFICATION • What procedure is in place ? • Who is responsible ? • Script provided ? • Process for tracking family members ?

  10. GOVERNMENT NOTIFICATION • What agencies do you notify ? • Who notifies the agencies ? • Notify the EM ?

  11. ROOM EVACUATION CONFIRMATION • What procedure is used to verify evacuation ? • How often are staff trained for this ? • Are others aware of your evac protocols ?

  12. RECORDS & SUPPLIES • Who transports resident records ? • How will you maintain confidentiality ? • How will you transport resident medications ? • How will you maintain controlled substances ?
