1. Lancashire Early Intervention Service Secondary Care Pathway for Clients Aged 16 to 35 Crisis Resolution & Home Treatment Service Community Mental Health Team or Primary Mental Health Team In-Patient Services EIS Referral • Review of Case Files • Discussion with referrer • May not involve client contact • Client does not meet service criteria Joint Assessment with referrer who has Care Co-ordination Clear Psychosis therefore client for Lancashire Early Intervention Service High risk mental state therefore client for Lancashire Early Intervention Service • EIS not appropriate. • Recommended for reassessment if new symptoms or concerns develop. • Care coordinated by referrer or back to GP • May include recommendation for care pathway.

2. Client is not offered care by EIS, responsibility remains with GP, however the following may occur:- • Recommendation for appropriate intervention if client needs other mental health services – with suggestion for re-referral to EIS if new symptoms of psychosis develop. • EIS will discuss with Duty Worker in appropriate single point of access mental health service and forward assessment documentation. This will be clearly marked as urgent or non urgent. • Client referred back to GP with detail of assessment outcome and suggestion for re-referral if new symptoms of psychosis develop. • Copies / letters sent to GP regarding action and client informed of outcome. Lancashire Early Intervention Service GP Referral Pathway • GP may telephone Lancashire Early Intervention Service for advice regarding the referral prior to making a formal referral to the service. • GP may use early detection tool via website prior to making formal referral to the service – please visit www.hope-health-recovery.org.uk. General Practice Referral (client aged 16 to 35) EIS Assessment Client is accepted into Lancashire Early Intervention Service Psychosis Group Client is accepted into Lancashire Early Intervention Service “High Risk Mental State” Group
