  2. Create and submit crash reports and citations https://traffic.ps.utah.gov/entry

  3. Login Enter UCJIS user name, agency, and password UCJIS access and application permissions required for use Contact BCI Helpdesk for access and permissions (801)965-4446 For training resources or questions, follow the link to the Crash/Citation page on the Highway Safety Website

  4. To create a new citation, select “Add” To create a new crash, first select “Crashes”

  5. Once on crashes, select “Add”

  6. After entering all information, you must “update” each page before moving to the next Information with a Bold heading is required This program uses a “tab” selection for data field “pages”

  7. If you fail to enter all required information on a page you will see an error message when trying to update

  8. If you try leaving a page without “updating,” you will receive a warning

  9. If the page is successfully “updated” you will see this message (which will disappear after a few moments)

  10. Adding a Vehicle(Crash Report) Step 1: Select “Vehicle” Tab Step 2: Select “Add Vehicle”

  11. Adding a Person(Crash Report) Step 1: Select “Person” Tab in “Vehicle” Section Step 2: Select “Add Person”

  12. Select Person Type People associated with a vehicle (driver, passengers, owner) can be added under the “Persons” tab within the main “Vehicles” tab If entered this way the person will automatically be associated with that vehicle

  13. Selecting the magnifying glass after entering driver license or license plate information will automatically populate information from the local database

  14. Enter Vehicle Information – Hit & Run Step 1: Enter information in required fields Step 2: Enter any known information 05 - Hit and Run - 0 - - - - Not Applicable - 1 99 - Unknown

  15. Enter Driver Information – Hit & Run Step 1: Enter information in required fields Step 2: Click Magnifying Glass and select vehicle

  16. After entering all crash information, check “Ready for Review” box on bottom of “Crash Details” page

  17. Searching for existing reports To search for reports created by another user within your dept, leave blank or enter their User Id Searching with your User Id will only produce reports you entered Users can enter search criteria in the other fields to narrow search results, then select “Submit”

  18. Search Results Users can verify a report has been submitted to the state database by checking for a PS Case #, which is assigned after a report has been received by the state. Reports can be edited, deleted (unless already submitted), printed, or submitted from this page. Users can also validate the report or print a Driver Exchange Form.

  19. To use an existing citation number, select the manual citation entry button – then enter the citation number in the Citation Number field Select court and enter date and time. ( Hitting space bar in time or date fields enters current time and date.) Creating a new citation

  20. Select a person type *Adult Subject is used for non-traffic offenses (no vehicle) *Juvenile Subject is used for non-traffic offenses for a juvenile *Driver Subject is used for traffic offenses (vehicle involved)

  21. Incident location *Enter Road Jurisdiction, City, County, and Location where offense occurred *If the violation did not occur within city limits, select “Outside City Limits” *Be sure to enter milepost number on roads with a milepost

  22. Typing a letter associated with a violation or the number will populate the associated list of violations for that jurisdiction Jurisdiction and Offense are used only to populate violation code drop-down list

  23. Selecting a code from the drop-down list will automatically fill in the Violation Code and Description fields Be sure to enter violation severity and other applicable information for that violation

  24. After entering violations you can select “List” to view all violations for this citation Select the “Edit” button to make changes to a violation
