ADULT DAY HEALTH CARE (ADHC) GOAL The Department of Veterans Affairs (VA) is committed to providing a continuum of long term care services for veterans at risk of institutionalization Provide that care in the safest and least restrictive environment Increase the number of ADHC in state home programs

    3. ADULT DAY HEALTH CARE (ADHC) CHALLENGES Barriers to increasing the number of programs: Current regulations resemble nursing home guidelines Current regulations limit the flexibility to determine the type of program to be established - Medical or Social Model

    4. ADULT DAY HEALTH CARE (ADHC) ACTIONS Revise the current regulations by: Changing language to remove the requirement to establish a program with care delivered by an inter-disciplinary health care team Eliminating the comprehensive Home Care assessment Changing the staffing & space requirement COLLABORATION = SYNERGY SVH & VHA Representatives work groups

    5. DOMICILIARY (DOM) GOAL The Department of Veterans Affairs (VA) is committed to providing a continuum of long term care services for veterans at risk of institutionalization Provide that care in the safest and least restrictive environment Increase the number of DOM in state home programs

    6. DOMICILIARY (DOM) CHALLENGES Barriers to increasing the number of DOM programs: Current standards are too restrictive and resemble nursing home care standards Lack of flexibility to determine the type of program to be established - Medical or Social Model

    7. DOMICILIARY (DOM) ACTIONS Revise the current guidelines to provide the flexibility to establish either a medical or social model program. Request your support in our efforts to increase the number of DOM programs. COLLABORATION = SYNERGY SVH & VHA Representatives work groups

    8. MINIMUM DATA SET (MDS) MDS data transmissions to Austin Automation Center Secure web portal established to capture data Alpha & Beta testing Conducted October 2008 thru January 2009 Quality Indicator (QI)/Quality Monitor (QM) Reports and Resource Utilization Group (RUGS) Reports Assist efforts to improve quality of care and services provided Collaboration SVH facilities participated in alpha and beta testing

    9. NATIONAL CONFERENCES State Veterans Home Training National training conference is planned for July 21 24, 2009 VA MDS 3.0 National Training National training conference is planned for October 2009
