  6. Dementia identification Dementia Data Harmonisation (DDH) Data on the Practice Dementia Registers are NOT a true reflection of the dementia patients DDH is a way to improve the quality of the data by looking at accuracy of coding and recording 6

  7. Dementia Data Harmonisation An automated and non-automated process Involves performing searches and comparing the results against the existing Dementia Register Improvements of 10 to 30% in Dementia registers Of financial value to practices – Enhanced Scheme and QoF (2015-16) 7
