MISSISSAUGA, ON, Oct. 18, 2018 /CNW/ Trillium Health Partners (THP) and SE Health (SE) are given the esteemed 2018 Minister's Medal Honouring Excellence in Health Quality and Safety. Founded by Robin Martin, Parliamentary Assistant to the Minister of Health Insurance and Longterm Care on October 17, both THP and SE were awarded this distinct honour due to their innovative cardiac program called Putting Patients at the Heart (PPATH). This year's theme, Innovating Integration, honours innovative campaigns undertaken across Ontario that look to achieve much better experiences for providers and patients, as well as better health effects together with better importance in healthcare delivery in Mississauga PPATH redesigned the maintenance journey for cardiac surgery patients by streamlining and simplifying use of follow-up maintenance for patients. It permits cardiac patients to return home earlier and with better affirms. Physicians report 96% satisfaction with the application and 98% would recommend the program to family and friends. It reflects the active engagement of patients at co-designing the ceremony alongside THP and SE Health, ensuring it integrates what matters most for them. PPATH has led to a reduction in post-surgery unexpected emergency department visits by 33%, and hospital re-admission speeds by 25%. Health-system savings of $1.7 million were achieved over two years. 3.5 times more patients received community care.
In addition to receiving this year's Minister's Medal, the program has been known across Canada receiving the Current Canadian College of Health Care Leaders, 3M Health Care Quality Team Award -- Improvement Across a Health System.
