  2. BACKGROUND • The are 4 community hospice centres , viz 1 . Chesterville Day Centre – opened 1998 2 . Phoenix - 2002 3. Inanda - 2004 4. Umlazi - 2004 Each centre has a full time professional nurse , caregivers and volunteers. We currently have 824 patients.

  3. OBJECTIVES • To decentralize hospice service to communities • To provide palliative care in the patient’s home • To provide other services, e.g day centre • To network with other organisations in the community • To empower and mentor communities

  4. PROGRAMMES • Palliative care – home visits by PN and caregivers • Day centre – support group , OT and skills • OVC – memory work , reach for a dream , teenage group ( life skills training ) • Bereavement – letters andby social workers through use of bereavement support group

  5. COORDINATION OF SERVICES • Weekly meetings at day centres • Daily allocation of work to PNs • Weekly submission of forms • Random visits – centres and caregivers- patients at home • IDT meetings every Tuesdays includingPNs,social workers and doctor • Community meetings - monthly

  6. CHALLENGES • Unemployment and poverty in the community • Crime • Lack of grants or discontinuation of grants • Work ethics – volunteers • Absenteesm • Forms for statistical purpose • Coverage – distance
