  21. UNCONTROLLED SEEPAGE DISPLACEMENT OF EMBANKMENT BLOCKAGE/DAMAGE TO CONTROL FEATURES EROSION FIRE (COAL REFUSE SITES) 5 CATEGORIES OF DEFICIENCIES
