  1. Requestors: • 4 required fields • The ones in RED text: • Status (Action Required) • Operational Area • Contact Info • Requested Mission • *Don’t forget Section#4 near the bottom, “Deliver this resource to” • It’s where you want the resources to show up!

  2. Status should initially be set to “Action Required” so CalEMA staff will know to coordinate this request. The other fields are auto-filled or will be set by CalEMA staff

  3. Choose County/OA from the drop down Where request is originating. If this request is ‘related’ to an ongoing Event or Incident choose it from the drop down list. If you can’t find it or don’t know it can be added later. Enter contact information for the person responsible for this request.

  4. Concise verbiage on the goal, what exactly needs to be accomplished. CalEMA staff and the responding agency will determine specific equipment & personnel best suited to meet the situational needs. Resource Type: Only if you know otherwise don’t worry about it. Needed by date: Absolute drop-dead date resources must be on scene. (within reason)

  5. This section is for the RESPONDING AGENCY to track/document the status of their resources, if they choose.

  6. REQUSTORS: VERY IMPORTATNT! This section details where the resources are needed. Be specific & descriptive; you know your area and the responders may not. Make it easy for them to get to you quickly.

  7. This section is for CalEMA staff. Choose your name from the list and the other fields will auto-fill. Document anything of NOTE that occurs during the course of this mission. Only important facts This is time/date/ID stamped. Hard-coded and can’t be deleted.

  8. CalEMA staff - select responding agency POC and Contact Info are vital. Enter Action Taken: Responding Agency can document activities & occurrences' for the course of the mission. Only important facts This is time/date/ID stamped. Hard-coded and can’t be deleted.
