  4. How to Make it Work? There are many components of Integrated working, but for it to be successful there may be barriers or challenges to overcome.....

Barriers / Challenges Language and Jargon Not Using Data as Intelligence Differing Priorities Geographical Boundaries Cost shunting Financial Challenges Different Leadership Styles Traditional Cultural Differences Need to Develop Relationships Lack of Sign up Across Organisation(s) Fear No shared System Professional Boundaries Not Sharing Information Unhelpful Behaviours

  6. How to Overcome Barriers / Challenges Some areas have come up with potential solutions to some of these barriers and challenges.....

Solution Suggestions • Language Differences and the use of Jargon • Have a clear vision from the start • Try not to use acronyms and write in Plain English wherever possible • Have patience and be accommodating with each other • Build relationships and acknowledge that we talk the same talk but use different phrases • Don’t be scared of asking if you don’t understand • Invite service users to sit on Boards to ask the question “what does that mean?”

Solution Suggestions • Cultural Barriers • Look at the governance structures and Leadership styles within organisations • Shadow roles/functions to promote learning and understanding • On the job learning and sharing lessons learnt/best practice • Look at shared metrics and drivers • Acknowledgement of levers in different organisations - maybe begin with a national lever like personalisation. • Follow the money through the organisation to begin conversations • Ongoing monitoring of services and use of stakeholders

Who’s Doing It Already? • Calderdale Council and NHS Calderdale have initiated an Intermediate Tier project because they recognised: • Life expectancy was worse than average in the area • There was a changing demography • Budget constraints • A rise in emergency short stay • Rising care home admissions • Rising home care costs • Feedback from people using services, carers and staff showed concerns

Who’s Doing It Already? • By taking a Single Commissioning approach, Calderdale Council and NHS Calderdale have got a clear vision: • Services design led by people’s needs and take account of their preferences • People are treated fairly and with respect • People only go into hospital when necessary • People only give their information once • People are assessed early so they can return to independence as soon as possible • Services focus on prevention and helping people stay at home when safe to do so

Who’s Doing It Already? • So what’s been achieved to date? • Clinical engagement throughout • Whole system outcomes identifed • Whole system data and evidence base collated to model demand and capacity • Public consultation • Stake holder workshops • Case for change accepted by providers • High level “To Be” pathway agreed • Draft single specification being prepared

Who’s Doing It Already? • Some of the Challenges / Barriers • Different structures • Differences in the way we work with our providers • Financial Modelling • IT systems/data sharing • Data collection • Communication/consultation - Different stakeholders and no shared system • Language and culture - Different language/concepts and no experience of working closely with each other

Who’s Doing It Already? • Positive Outcomes Seen Already Include • Shared vision for the Tier • Single health and social care outcomes framework, with an aligned set of outcomes • Metrics under development • Rapid improvements in services through increased focus on them • Integration pilot – admission prevention team • Third sector realising added value • Better understanding of how the Calderdale Whole System works • Better understanding of how to make single commissioning a reality
