1. Re-forwarding the original FAP - Abrasion/Laceration, that occurred in our Onshore Group (STx), January 10th, 2005, notice with additional pictures to clarify what is thought to have logically transpired. Original thoughts and perceptions were that the pressure washer wand coming into close proximity of the EE's boot had ruptured the boot and caused a laceration to the EE's toe, even while the boot was submerged in wash water. After further investigation, it is now perceived that the boot worn by the employee was actually damaged prior to the incident. When the pressure washer wand was inadvertently crossed over the boot, the small damaged area of the boot allowed the HP water to contact the toe area. Attached pictures show the two boots worn by the EE. The affected boot picture showsa pen pointing out the toearea with a small, nail-size hole that is positioned right over the toe that was injured.Second Pictureshows the other boot worn by the EE that was damaged, under controlled conditions, by application of HP water without being submerged in water. Incident summary will be shared with roustabout contractors we employ in our STx Operations and ask thattheir JSA's address the issue of damaged PPE (boots).
