Your Leaders Will • Lead the organization to achieve more and mean more to our patients, our constituents and our country • Work with you, listen to you and provide what you need to successfully navigate through a time of real change, growth and economic uncertainty • Recognize the size of the changes we’re making and the challenges we’re facing

Transition = More Programs Transition to Divisions means: • Greater ability to meet the needs of patients • Targeted and focused programs • Program Partnerships among Divisions and between Divisions and National Headquarters • More funding of the best kidney research in the US • We will retain great traditional Affiliate programs and enhance them and bring them to the whole country

Transition = More Responsibilities Our Board, The Transition Strategies Group and the staff leadership are committed to: • Meet the inevitable challenges that this major change means for all of us • Carefully monitor the financial implications of the transition • Provide all of you, Divisions and Affiliates, with the support, information, assistance and ideas it will take to make NKF more successful • Increase the financial strength, program effectiveness and reputation of NKF

Transition = More Money Transition to Divisions will mean: • Creative and Productive Fund Raising Partnerships developing among the Divisions, National Headquarters and between Divisions • Major donors can be identified and solicited though a national / local cohesive effort • A solid Planned Giving Program can be developed and rolled out around the country • Consistent programs and branding

Transition = Productive Volunteers • Active engaged volunteers are vital in Divisions • Division Chairs are pleased with their role and are accomplishing more than ever • Division Boards and Medical Advisory Boards are able to focus on real progress • Community visibility is increased • Volunteers still develop priorities, evaluate programs and plan for future growth • Division volunteers will have a real voice at NKF

Transition to Divisions • The objective is to be a stronger, more effective organization in meeting the needs of our patients and meeting the challenge of the huge public health problem that kidney disease is in the United States • This approach is similar to our “competitors”, more responsive, more flexible and agile and more relevant to fulfilling our mission

The Transition is Working • My personal goal and my hope is for the map to be all one color • We respect the voluntary nature of the decisions you all have made or will be making • We realize a few of you want to remain Affiliates • But our Board and Staff are committed to making sure the outcome of each decision to become a Division is so successful that making it was obviously the right thing to do

  10. We Are Building On Success • 2008 has been a very good year for NKF • We are: • raising more money • doing more programs • funding more research • identifying more early kidney disease • communicating more with each other • fundamentally changing the organization for a better future

NKF’s KEEP Program • Nearly 120,000 individuals participated in the United States • >2,500 participants per month, and >2,500 programs conducted to date • >20,000 doctors enrolled in the KEEP database • Established expertise and experience • Validates our position as the leader in CKD • New Longitudinal Component Launched in June

Growth in 2008 KDIGO & KDOQI • NKF Signature Programs to improve outcomes through clinical practice guidelines • KDOQI is our original brand and continues • KDIGO develops guidelines globally, as they should be done • Our NKF Board of Directors funded the Guideline on Care of the Transplant Recipient

  18. Best Series of Amateur Golf Tournaments for Charity in the US • Both NKF Golf Classics and Cadillac Invitationals done by NKF are growing. Over 75 tournaments nationwide • Brought NKF $3.5 million in 2008 • Continuing to expand sponsorship search • Ping Sponsorship is growing • Relationship with Golf Digest continues • Continue to upgrade prizes and golfer gifts • Solid long-term relationship with Pebble Beach

Kidney Cars • “Kidney Cars” is 25 Years Old • Revenues are trending back up • FY 2008 was 18%higher than FY 2007 • Gross revenue was $12.9 million • Net revenue was $9.67 million • That is a profit ration of 75%  • NKF public policy staff and volunteers will go back to Congress in 2009 to address tax regulations on donated cars • Thanks to increased internet marketing and vendor negotiations, net results are strong, despite the slowing economy

A new Kidney Cars Sweepstakes began Sept 1 to celebrate the 25th Anniversary and generate additional revenue • This will help increase visibility, stimulate revenue and reward our generous donors • Any car donated between Sept 1 and Dec 1 can is eligible for prizes • Grand prize is Pebble Beach trip & chance to win $25,000!

Your Successes • Delaware Valley > $500,000 this fall in walks • Kansas City > $65,000 on their first walk • George Lopez Celebrity golf tournament in LA was a resounding success • Our South Carolina Affiliate was featured in Time Magazine • After “researching charities” and hearing about NKF programs, a donor walked into our office in Dallas with a check for $200,000

  22. Your Successes • “Coffee House Conversations” to support patients held in 32 cities • “Grand Rounds” professional education programs held in 5 cities in New York • Post-KEEP education programs pilot tested in 5 cities • CKD Task Forces in operation in 12 states

NKF’s Vision Your National Kidney Foundation has the vision to: • Adapt its structure to a changed environment • Adapt its mission to recognize that we have 26,000,000 Americans to serve • Adapt its focus to include major programs on kidney disease as a public health problem • Adapt its scope to reflect the fact that the world is getting smaller

NKF’s Strategic Plan 2007-2010 THE TRANSFORMING GOALS Early Transplantation Branding Global Vision Transition to Divisions Patient Empowerment CKD Detection & Treatment • The Plan remains a priority • 85% of the first year items in the plan have been completed • Remaining items are being pursued

KEEP • KEEP staff, led by SVP Gigi Politoski and KEEP Director, Monica Gannon, is expanding to devote highly qualified and experienced people to the program • Leslie Pratt Dyer is working with Affiliates and Divisions • High quality and compliance is absolutely essential • Fiona Kuan, KEEP’s Quality Control Director, will work with Affiliates and Divisions and will visit KEEP events • Headquarters improvement priorities: • shorter follow-up timelines • use of a dedicated call center • smooth integration of the longitudinal component • enhanced use of the KEEP data

The KEEP Development Fund • 31 grants were approved in 2008 • 14 Affiliates and 12 Divisions received funding • New areas of the country have been served • All Affiliates and Divisions can expand KEEP in their area with financing from this Fund • Call Gigi or Monica and talk over your ideas • Be creative, use the money to benefit your Affiliate or Division and KEEP • A efficient way to raise money to expand KEEP • Depending on results, more funding next year

KEEP • Longitudinal Component • Since June, 37,000 past participants have received an invitation to return • From June 15th through early September, 1,700 people were screened; 20% were returning participants • Follow-up on test results is critical • Goal is to increase the number returning • Increased value to participants and research • Enhanced Web site for participants who can or have returned and their physicians

  31. KEEP Future focused on: • Strategically Planned Growth • Continuous Quality Improvement • Data Value and Use Enhancement • Longitudinal component • Value of Quest Labs • Post event education and awareness • Global expansion

Non-KEEP Detection Programs • Program can identify early CKD without using the full formal KEEP protocol • No blood tests, different inclusion criteria • No data in the KEEP ADR • Suitable for some locations or events where KEEP is impossible such as NBC Health Fairs, AMA meeting, Grand Central Station… • Doing this can be a good intake system for KEEP • NKF provides insurance if you inform us

CDC Detection Program • NKF efforts in Congress brought a CKD program to the CDC; one element of that is a program, based on KEEP called CHERISH • Will include 800 participants • Will validate KEEP and stimulate further state and local government interest in CKD and early detection • CHERISH sites are in New York, California, Florida, Minnesota • There may be a phase two program in more states • Rebecca Seeman (Study Director) will be working with Affiliates and Divisions who will be conducting CHERISH programs

KLS Continues to Grow • More than 26,500 professionals have taken KLS Continuing Education courses • New topics in HIV, Proteinuria, Contrast Imaging, Kidney Biopsy, Hepatitis C • 6 Issues of Hope Street published • New multi-disciplinary curriculum for non-physicians • Patient education published in 2008 in 11 languages • Online formats are the fastest growing • NKF’s CKD library is the largest and best in the world

KDIGO Continues to Grow • KDIGO Guideline on Hepatitis C and kidney disease is being noticed around the world • KDOQI’s Commentary on the guideline is in press • KDIGO’s Guideline on the Care of the Transplant Recipient will be available for public comment this fall and published in early 2009

  37. KDIGO Continues to Grow • KDIGO’s next Guideline will be on Mineral and Bone Disorders and is currently under public review • KDIGO’s Guidelines after that will be on: • Acute Kidney Disease • Glomerulonephritis • Anemia • Cardiovascular Disease and CKD

  38. KDOQI Continues to Evolve • KDOQI is a strong brand • NKF reputation is enhanced by KDOQI • New KDOQI features four major areas of focus: • US commentaries on KDIGO global guidelines • National and regional conferences on important topics • Public Policy • Research

NKF in Washington, DC 2008 Public Policy Priorities Pass the Kidney Care Quality and Education Act to improve dialysis services and payments (HR1193/S691) Create a new Medicare benefit for pre-dialysis education (HR1245/S. 432, also included in HR1193/S691) Extend immunosuppressive drug coverage for kidney transplant recipients (HR3282/S. 2320) Expand the CDC kidney early detection program Increase funding for kidney research at the National Institutes of Health Expand the Congressional Kidney Caucus 26

  40. NKF in Washington, DC Kidney patients scored a victory on July 15 when Congress overrode a presidential veto and passed the Medicare Improvements for Patients and Providers Act of 2008, H.R. 6331 NKF People Like Us Representatives at the Capitol

NKF in Washington, DC • Our bill, the “Kidney Quality and Education Act,” included in MIPPA, passed dramatically • The bill provides: • Annual update of the dialysis reimbursement composite rate • A program of quality assurance to protect patients • And, it provides NKF with the opportunity to establish its leadership by designing the program to educate patients at Stage 4

Keeping You Informedand Involved • Join our growing Take Action Network, which informs our constituents of a need to immediately contact their representatives • We now have 5,669 members in 434 of the 435 Congressional Districts in every state. • Since the inception of the network 14,852 messages have been sent! • Joining is easy at www.kidney.org/takeaction

  43. “People Like Us” Washington, DC Fly-ins (2008) • NKF visited more than 250 Congressional Offices in 2008 seeking improvements in the Medicare ESRD Program and increased funding of the CDC kidney disease program • March 2008 “World Kidney Day” – 23 “PLU” advocates visited 80 Congressional offices • May 2008 – 40 “PLU” advocates, along with Dialysis Patient Citizens, visited over 150 Congressional offices • Five “Fly Ins” scheduled for 2009

  44. “People Like Us” Activities for Affiliates/Divisions • “PLU” can provide training for NKF Affiliates/Divisions on: • Advocacy • Federal and state public policy issues • Patient advocate recruitment • Effectively working with local legislators, media, community groups • “PLU” will be available to help Affiliates and Divisions with local or state public policy activities
