Identity Theft Flowchart Suspected ID Theft Employee notifies Department Director Department Director notifies CQO Verify ID theft has occurred NO CTS entry not required; continue patient processing as usual YES CQO entry into CTS Notify CEO Involve HIM and PFS Director to ensure acct is changed to IDtheft… follow HIM and PFS policies Determine whether patient info is combined with that of thief; or, disclosed to inappropriate party NO YES Involve HIM and PFS Directors. Follow HIM and PFS policies for changing acct to IDtheft… Inform CQO if victim’s information has or may have been inappropriately disclosed Work with hospital legal counsel regarding notification of patient/law enforcement.
