  1. StarForm Author Tracking • If there is more than one author who contributes to the Admission History (or any StarForm), you can view the full electronic “paper-trail” by following the directions on the next 3 slides. • Look at the header at the top of the form for cues regarding single or multiple authors.

  2. From Actions Menu, select All Documents and scroll down documents to find the Admission History form. Click on name of form to open.

  3. If the form has a single author, the header will look like this with created by … and electronically signed by being same person. If you click the hyperlink of the author’s name, in the frame below, you see the form again with a red header saying “Current (and only) version:”.

  4. If there is more than one author on the form, there will be no hyperlink of author’s name at the top of the form. Instead, scroll to the bottom & you’ll see a hyperlink for “Signatures”. When you click on Signatures, in the frame below, you see every field of the form, responses, date, time & by whom the response was entered.
