  3. Section A • Must be completed. • Used more as a checking tool. • Must include verification code, amount of parking charge and any sum paid. • Confirmation that evidence has been sent to Appellant. • Page count to double check we have received all evidence.

  4. Section BCase Summary & Rules/Conditions MOST IMPORTANT SECTION – YOUR OPPORTUNITY TO PUT YOUR CASE! • Need to address issues raised by Appellant on appeal form. • Provide conditions etc you allege have been breached. • You can refer to any correspondence which you must attach – just to emphasis your case. • If in doubt include it!

  5. Section CParking Charge Notice and any notes • MUST always be included. • If copy, must include all relevant information (everything that’s on the ticket). • If the operative makes any notes (i.e. ‘I handed it to the driver’, times observed, activity noted etc), these must be included.

  6. Section DRegistered Keeper Details/Liability trail This section shows how you claim the appellant is the person liable. For example; • Keeper details obtained from DVLA including date obtained. • Notification of driver by keeper.

  7. Section EOriginal Representations and NoR • Must send original reps. This must include any evidence attached such as tickets, images, witness statements, maps etc. • Any additional correspondence in respect of the PCN must also be included. • A full copy of the Notice of Rejection must be included. It is important that the entire document is sent.

  8. Section FImages, plans etc • Pictures taken at the time of PCN issue. For example, PCN on the vehicle, signage and position of vehicle. • Plans of car park. • Copies of conditions of parking. For example, letter format or a printed version of a sign. • Any moving images/voice recordings. • Print outs of automated payments.

  9. Section GOther evidence Anything else you want the Assessor to consider that isn’t included elsewhere.

  10. And finally…. • If you are sending evidence by post, please ensure that you separate each case.
