  4. What is required for a substantive amendment? • Public hearing • Modify post award public briefing notice CHIP Financial Manangement

  5. What must be included in amendment request? • Cover letter from Chief Elected Official • Public hearing notice and copy of minutes • Narrative describing change (remember to discuss total project (not just specific activities; not just CDBG funded portion) • Letter from engineer • Revised map • Revised forms 2, 6, and 8 CHIP Financial Manangement
