  3. 27/May Haima to Qarn Alam black top-road Two pedestrians wanted to cross the road. One stopped and looked left and right. The other stepped onto the road without looking and was hit by a passing Landcruiser. He died on the spot.

Discuss in your team (27/May fatality):If you were the pedestrian that died: - How could you have prevented this accident ?- Why, do you think, did he not do these things ?If you were his colleague (the one that stopped):- How could you have prevented this accident ?- Should you warn a colleague of an obvious Hazard ?If you were the driver of the landcruiser:- How could you have prevented this accident ?- Would a warning signal with the horn have helped ?- Would you have thought that the man would cross without looking ? Would you have been prepared ?

  5. This large truck saw a small canter moving to his side of the road. He flashed his lights and started pulling off. The canter did not react and hit the truck. 5/June Saih Rawl to Qarn Alam black top-road

  6. The canter driver died on the spot.The 2 passengers survived: they were wearing seatbelts. The truck driver was not hurt. 5/June Saih Rawl to Qarn Alam black top-road

  8. This vacuum truck was off-loading oily waste.Discharge blocked. The helper removed the hose and tried to unblock with stick through the valve. Pressure pump was kept running. 8/June Rasha waste management area (Rima)

  9. Suddenly the rear closure slammed open with enormous force. The helper, who stood behind it, was thrown 13 m away. He died on the spot.It is not yet clear why the closure/door failed: - Faulty Relief Valve? - Faulty pump pressure switch? - Internal explosion? (no earthing was used) - Bolt failure (8x 2”dia)?   11. Truck left the road down the hill here 10/June Salalah to Thumrait mountain road A truck, transporting 20.000 liter diesel, came down too fast through this right hand turn. It rolled over and went through the guard rail into the wadi below.

  12. The truck rolled several times. The driver was thrown out and died on the spot.Diesel spilled into the wadi.

Discuss in your team (10/June fatality):If you were the truck driver: - How could you have prevented this accident ?- Why, do you think, did he not do these things ?If you were the supervisor/manager of the truck driver:- How could you have prevented this accident ?- How do you ensure the truck is in good condition ?- How do you ensure your driver is trained and competent ?- Why would your driver drive too fast ?- How do you STOP him driving too fast ?

  14. Some learning points from these 4 fatal accidents:- On long open road: warn a lone pedestrian with the horn. He might not have seen you!- A car/truck towards your side of the road: blast your horn, slow down, move off the road.- As a passenger: do not distract the driver. - As a driver: keep concentrated. Tired ?: STOP and rest.- Blockage in pressure system: shut down and get help from a supervisor or qualified operator. - Earthing systems: where provided to eliminate sparks: USE THEM - Driving down a steep slope: engage low-low gear. Go slow!!- Seatbelts save lives: wear them.
