1. Fellowship Advancement Made Easy (FAME) By Marc Shabot, Former Texas Southern Governor Prerequisites 1. ABIM, RCPSC, or AOBIM certified† 2. Licensed practitioner 3. Formal training completed, Member at least 2 years, in practice or teaching for at least 2 years 4. Proposed/seconded by 2 Masters/Fellows 5. Commitment to lifelong learning and professional development †May be waived in extraordinary circumstances

2. Then, Qualify by 1 of the 4 Pathways* Pathway 1 - Academician • Holds academic appointment as teacher, researcher, administrator, writes scientific papers, scholarly reviews, book chapters, etc. Pathway 2 - The Scholar/teacher/ Multiple Certification • Active in community as teacher in Continuing Education and Professional Development activities • Re-certified or dual boarded, or MKSAP for score

3. Pathway 3 - The Active ACP-ASIM Member • 10 years membership in the ACP-ASIM • Active in ACP-ASIM meetings and Committees at local and national level • Pathway 4 - The Senior Physician • May have been a longstanding Member or have joined ACP-ASIM later in life. Has demonstrated longstanding professional activity in the community, in teaching, in patient care or in service; acts as a role model for other physicians and health professionals • Note: Community service, especially the voluntary provision of medical care, and ACP-ASIM activities significantly enhance the likelihood of advancement, under all four pathways.

Fellowship Pathway 1 — The Published Academician • Demonstrated skill in written medical communication • Ability to organize scientific data clearly and to communicate these data in writing • At least two published articles since the completion of training • Contributions to multi-authored articles must be noted in bibliography even if candidate is listed as first author • Reprinted articles may be requested by Credentials Subcommittee • Peer-reviewed journals are given more weight

5. Fellowship Pathway 2 — The Physician with Multiple Certifications or Advanced Degrees • Significant and continuing educational activity, such as • Recertification in internal medicine; or • Subspecialty certification; or • Participation in the Medical Knowledge Self-Assessment Program® (MKSAP) for CME credit; or • Certificates of special competence in adolescent medicine, critical care medicine, clinical cardiac electrophysiology, geriatrics, or sports medicine; or • Advanced degrees such as PhD, MBA, or MPH • Teaching, both institutional and community-based Note: Exceptional candidates with outstanding scholarly and/or teaching and/or community activities may qualify under this pathway even without fulfilling the recertification, subspecialty certification, or MKSAP requirement.

Fellowship Pathway 3 — The Active ACP Member • At least five years of ACP membership • Substantial participation in ACP activities, such as • Annual national meetings • Local chapter meetings • Postgraduate courses • Development of educational materials exhibited at ACP meetings • National or local committee, council, or task force participation Note: Weight is given to the completion of the Medical Knowledge Self-Assessment Program® (MKSAP) for CME credit.

Fellowship Pathway 4 — The Senior Physician • Distinguished professional activity or prominence in teaching, patient care, or professional service over many years • Designed for senior practitioners who have not been active in ACP or who are not board certified Note: Weight is given to the substantial teaching contributions, scientific or scholarly presentations, or the organization of scientific or clinical training programs on a continuing basis. Candidates should document active participation in hospital or civic activities

Advancing to Fellowship All applicants must meet the basic eligibility requirements for Fellowship, summarized below and detailed in the Membership Information Booklet In addition to the basic eligibility requirements, there are four ways to qualify for Fellowship. These are not mutually exclusive, and are provided only as guidelines to the types of activities necessary to attain Fellowship. Some candidates will be accepted for Fellowship based upon a combination of activities.

Basic Requirements (Part 1) • Certification in internal medicine or neurology • Professional activity in internal medicine, a combined internal medicine specialty, the subspecialties of internal medicine, or neurology • Experience in practice or in an academic position two years since the completion of residency • An active medical license in good standing (if in • clinical practice) • ACP membership (Associate or Member) in good • standing for two years since completion of • residency training

Basic Requirements (Part 2) • Continuing professional development and • scholarly activities, including continuing • medical education as a student or teacher • Teaching (institutional and community-based), • hospital committee work, and/or volunteer • and community service, especially in the • voluntary provision of medical care • ALSO Must Subscribe to the ACP Pledge in • writing.

Learn More About Fellowship Fellowship is an honor achieved by those recognized by their peers for personal integrity, superior competence in internal medicine, professional accomplishment, and demonstrated scholarship. It may be achieved by either advancement from ACP Membership or, in exceptional cases, by direct election.

17. Learn More About Fellowship (cont.) Honorary Fellowships are granted to presidents or their equivalents of medical societies abroad who attend ACP’s annual Internal Medicine meeting. Fellows are authorized to use the letters FACP (Fellow of the American College of Physicians) in connection with their professional activities for as long as their membership remains current. They have the right to vote, hold office, and sponsor applicants for Membership and Fellowship.

19. The Meaning of Fellowship • Fellowship in the College is an honor. It is a mark of esteem from colleagues who recognize accomplishments and achievements over and above the practice of medicine. • A physician’s achievements and accomplishments could be limited by the area where he or she practice or by the available resources. The most important considerations for Fellowship are excellence and contributions made to both medicine and to the broader community in which the internist lives and practices.

20. The Meaning of Fellowship (cont.) Candidates for Fellowship should be recognized for exemplifying the mission and goals of the American College of Physicians in their professional lives. The items listed below are considered important in indicating that individuals are ready for Fellowship: • Upholding and practicing the highest clinical • standards and ethical ideals • Providing leadership at the community, • regional, or national level in matters • relating to health, citizenship, and social • Improvement

The Meaning of Fellowship (cont.) • Providing education and information to others, including students, residents, fellows in training, other practicing physicians, or allied health professionals • Advocating responsible positions in health and public policy through work on committees, in hospitals, in other medical societies, and in the community • Providing voluntary medical care and working on community service projects related to medicine or a broader area
