  5. What is Medicare? • Medicare is a complex federal health insurance that covers over 50 million Americans • Seniors (age 65 and older) • Permanently Disabled • End-Stage Renal Disease (ESRD) • Amyotrophic Lateral Sclerosis (ALS/Lou Gehrig’s Disease) • Exposed to Environmental Health Hazards

  6. Parts of Medicare • Part A • Inpatient Care (Hospital Services) • Part B • Outpatient Care (Doctor's visits and clinics) • Part C • "Medicare Advantage" • Offered by private plans (HMOs, PPOs) • Combination of Part A, B, and D • Part D • Prescription Drug Coverage

Medicare Part D • Prescriptiondrug coverage is part of the Medicare benefit and provided through Part D • Many aspects of Part D change each year, and this is where we come into play! • We assist Medicare Beneficiaries in optimizing their Part D plan and thereby minimizing out-of-pocket costs through targeted assistance. • And we are here to serve communities across Northern/Central California….including YOURS!

Howcan we help patient with Part D? • Medicare Plan Finder Tool • Evaluate all of the available Medicare Part D Plans based on a beneficiary’s medications and personal preferences (e.g., pharmacy) • Save them $$$ on their prescription medication costs • 9 of 10 can potentially save $ • On average, ~$700/person annually

What other service do we provide?Disease State Screenings/Testing • Vaccine Administration (flu, pneumococcal and Tdap) • Asthma and COPD testing • Bone Density testing • Anemia Screening • Falls Risk Assessment • Blood Pressure Testing • Body Fat Measurement • Cholesterol Testing • Diabetes Testing • Memory Decline Screening • Depression Screening • And more…

Our Medicare Part D Program Accomplishments In the past 6 years: • 62outreach events in 15 different cities in California • Assisted 2,215beneficiaries with their Medicare Part D plan • Helped 600(27.1%) beneficiaries in their native languages • Served 742 (33.5%) low-income beneficiaries • (e.g., Medi-Cal or Low-Income Subsidy Recipients) • Medicare elective pharmacy students have donated 6,048 hours of their time to help beneficiaries better understand their Part D plan and medications

Cumulative Result of Our Efforts Total estimated out-of-pocket savings on Part D drug costs as a result of student pharmacist interventions=$1,579,429 Severe medication related issues identified in 238 (16.6%) patients provided MTM services In 2012, prescribers accepted our recommendations for 46 (74.2%) patients in whom severe medication related issues were identified

So, how can you become involved? • Shadow student pharmacists working under pharmacist supervision • Serve as Interpreters • Mandarin Cantonese Spanish • Russian Vietnamese Tagalog • Sign Language • Spread the word about our events in your local community! • Flyers, senior homes, senior centers, ….invite seniors and other Medicare beneficiaries who can benefit!
