  2. What’s it all about? • Recognising what information is presently available to patients and relatives. • Discovering how much information was available and how it was perceived.

  3. What was our aim? • To improve the patient experience with information provided • To promote person centred care through effective information

  4. Getting Started • Discussion at OPAC** meetings regards information available to patients and relatives. • Recognising the need to review its effect • Utilisation of the effective practitioner website and grant to facilitate any improvement process **Older People in Acute Care

  5. Information and the Effective Practioner initiative. • Site used to improve staff awareness and understanding of person centred care. • Self assessment tools completed by team members. • Used site as an inclusive medium • Used materials from EP** site as assessment tools for relatives to use. **Effective Practitioner

  6. What did we do? • Took photographs of existing information displays- ward entrances notice boards • Undertook relative opinion • Explored communication opportunities- such as visiting times

  7. Outcomes of each of our actions: • Relatives felt it was information overload. • Few relatives read the information boards outside the wards. • Through PDSA- re-visited afternoon visiting

  8. Challenges of the project. • One member had to leave the team due to relatives ill health. • Gaining feedback from relatives regarding visiting times. • Annual leave and job changes. • Achieving our goal.

  9. Further challenges experienced: • Reached out to other NHS trusts and internal agencies. • Learned some information not updated • Obtaining responses • Developed a good working relationship • Promoted self development in others

  10. Where do we go from here? • Discussions with public relations regarding a trial to remove an information board • Attempt to use language that fosters an inclusive attitude. • Cascade findings to others at next OPAC meeting
