  4. Accomplishments • Regional Collaboratives • Dining Pilots • CARE Recommendations • Statewide Conferences • Webinars • Representing California at Pioneer Network, and other “person-centered” conferences

  5. CCCC’s Role • Overall Lead • Convene stakeholders • Keep activities on track • More Active Role • Training / Education • Consumer Awareness

  6. Bridge Grant • Six months • April to September, 2013 • Keep partnership moving forward, while obtain more secure funding • June: phone meeting • July: in-person meeting • August: in-person meeting • September: phone meeting • Funded by California HealthCare Foundation

  7. CMP Grant Proposal • Two Years • Hire program director • Workplan • Lead agency • Consumer Awareness • Training/Education • Facilitate workgroups • Enforcement • Informed Consent • Seeking funding from federal CMP funds

  8. Collaborative Model • CCCC funded to organize and facilitate the “Partnership” • Whenever possible will work with and build on the efforts of other stakeholders • All bring different by needed expertize and avenues of influence
