Service Hour Forms • Must complete 10 hours of volunteer service • 5 hours completed on campus; 5 hours completed off campus • What are some good ways to get hours? • Servant for a Day • Friends of the Night People • SABAH • St. Luke’s Mission of Mercy • Response to Love Center • Volunteer teach/aide religious education classes for younger grades • As different service opportunities become available, we will notify you via email and the parish bulletin

Mass Reflection Sheet • Mass Reflection Sheets are to be turned in each month from September 2014 to May 2015. • It should have a record of at least four Masses (in an average month with four weekends). • Some months will have five weekends, and some months will have a non-Sunday Holy Day of Obligation. • When these occur, record them on the back of the sheet. • Mass Reflection Sheets are due at the end of each month.

Saint Reports Candidates are asked to write a report on the saint they have chosen as their patron.  Minimum of one full page of single-spaced text, maximum of three pages.  12 point font size, single-spaced; no handwritten reports. Candidates may use the internet, the public library, Christ the King Seminary library, and their school’s library. Make sure the final product is the candidate’s own work and not copied from a website or a book. Plagiarism is very sinful & will result in failure of the assignment and disciplinary action for cheating. Sentences quoted in the report must be clearly indicated by quotation marks, with the author and sourced cited in the text. Due Date: December 1, 2014

Sponsor Information Sheet • When considering the choice of a sponsor, the following criteria must be met: • The sponsor should be a faithful member of the Catholic Church who is an active participant in the Mass, attends every Sunday, and receives the Sacraments often. • The sponsor must have already celebrated the Sacraments of Baptism, Communion, Penance, and Confirmation. • The sponsor must be in good standing with the Catholic Church and free to celebrate the Sacraments (for example, a person who has remarried outside the Church cannot be a sponsor). • A sponsor may be one of the candidate’s baptismal godparents. This would be a follow-up to the relationship and responsibilities that flow from Baptism. • According to the directives in the Code of Canon Law #893, neither the father nor the mother of a candidate can be a sponsor. The sponsor must be at least 16 years of age. • Due Date: December 15, 2014

  11. Letter of Intent As part of their Confirmation preparation, Confirmation candidates are asked to type a formal letter to our Bishop, Richard J. Malone, expressing their desire to receive the Sacrament of Confirmation. In this letter, candidates are to include a statement of intent to grow in love and knowledge of the Catholic faith and to advance the cause of Christ and the Church in the world. Letters should be about one page in length, typed, and grammatically correct. All letters will be reviewed by the Faith Formation staff and the pastor, Rev. Arthur Mattulke, before being sent to Bishop Malone. If a letter is lacking in any of the guidelines indicated on the Letter of Intent sheet, the candidate will be asked to rewrite the letter until it is of acceptable quality. Letter are to be turned into the Faith Formation office no later than March 1, 2015

  12. Absence Policy • My child will not be able to make class on Sunday (Group Peter/Paul) because of ‘excuse X’. Can he/she attend the other class on the following Sunday (Group Peter/Paul) and not be marked absent? • As long as you contact the office by either phone/email before the scheduled class, this can be arranged • The candidate should be the one to contact us! • If the candidate can’t make to either class for Group Peter/Paul, we will allow a parent to attend the class in your child’s absence; the parent will be expected to review the material in the class with their child afterwards

  13. Decision Point Confirmation Program • Videos for all sessions available online (www.dynamiccatholic.com/confirmation/view-program/). • Parents and sponsors can view the same material candidates will be taught. • Decision Point app is also available for download.

  14. Weather Cancellations In the event of bad weather, we will use TV-channels 2, 4, & 7, the website, social media pages, and WBEN 930AM to announce cancellations of classes Do not call the rectory!
