  3. Rheumatoid Arthritis • A chronic syndrome characterized by nonspecific, usually symmetric inflammation of the peripheral joints, potentially resulting in progressive destruction of articular and periarticular structures, with or without generalized manifestations.

  4. Rheumatoid Arthritis Criteria for Classification Any 4 of the following criteria must be present to classify patients as having RA: • Morning stiffness for greater than 1 hour. • Arthritis of hand or wrist joints. • Symmetric arthritis of at least 6 weeks duration. • Rheumatoid nodules. • Radiographic hand changes must include erosions or unequivocal bony decalcification.

  6. Gouty Arthritis A recurrent acute or chronic arthritis of peripheral joints that results from deposition in and about the joints and tendons of monosodium urate crystals from supersaturated hyperuricemic body fluids.

  7. Gouty Arthritis • Gout usually involves only one joint. • May be precipitated by minor trauma, overindulgence in purine-rich food or alcohol, surgery, fatigue, emotional stress or medical stress. • Gout is one of the most controllable metabolic diseases.

  8. Gouty Arthritis Therapy Therapy includes: • Elimination of alcohol intake • Low purine diet • Achievement of ideal body weight • Liberal consumption of complex carbohydrates • Low fat intake • Low protein intake • Liberal fluid intake

  12. Osteoarthritis Degenerative joint disease • An arthropathy with altered hyaline cartilage and characterized by loss of articular cartilage and hypertrophy of bone, producing osteophytes.

  13. Osteoarthritis • Most common articular disorder • Begins in second to third decade of life and is extremely common by age 70.

Infectious Arthritis • Acute infectious arthritis (95% of cases) is caused by bacteria or virus. • Neisseria gonorrhoeae is the most common bacterial cause in adults. • Non-gonoccoal arthritis is usually caused by staphylococcus aureus (45%), streptococci (9%0 pseudomonas aeruginosa (40%) and serratia marcescens (5%).

Risk Factors for Infectious Arthritis Advanced age (50% > 60 yr), alcoholism, anemia,chronic illness (liver or lung disease),diabetes, hemophilia, immunodeficiency including HIV, immunosuppressive therapy, including corticosteroids, intravenous drug use, malignancy, prosthetic joint implant, renal failure, rheumatoid arthritis, sickle cell disease, skin infections, SLE.

Psoriatic Arthritis • An inflammatory arthritis associated with psoriasis of the skin or nails; a negative test for rheumatoid factor; and HLA-B27 antigen in some patients especially when the spine is involved. • Psoriatic arthritis occurs in 7% of psoriasis patients with an increased prevalence in some AIDS patients.

  18. Controllable Factors Contributing to Psoriasis • Incomplete protein digestion • Bowel toxicity • Impaired liver function • Alcohol consumption • Excessive consumption of animal fats • Nutritional deficiencies • Stress
