Rehabilitation and Discharge Destination Interactions on Inpatient Functional Abilities

This study aims to examine how in-hospital rehabilitation and discharge destination interact to influence patients' subsequent functional abilities. The study evaluated 835 stroke, orthopedic, and disuse syndrome inpatients who underwent rehabilitation between February 2008 and December 2010. Results showed significant interactions between rehabilitation potential and discharge destination, influencing functional ability after hospital discharge for stroke and orthopedic patients. These findings have implications for deciding the appropriate discharge destination and highlight the importance of transition-support programs for rehabilitation patients.

  2. Aim • Examine how in-hospital rehabilitation and discharge destination interact to influence patients’ subsequent functional abilities. • Relevance • No studies have evaluated how functional abilities following discharge are influenced by these two factors.

  3. Method • Participants: • 835 stroke, orthopedic, and disuse syndrome inpatients who underwent rehabilitation Feb 2008-Dec 2010 at Japanese hospital. • Hierarchical linear regression analysis in each condition subgroup estimated interaction effects between rehabilitation therapy and discharge destination on subsequent functional ability. • Dependent variable: • Functional Independence Measure (FIM) score at 3 mo after hospital discharge.

  4. Results • Stroke and orthopedic patients: • Significant interaction between rehabilitation potential (measure based on FIM effectiveness measure) and discharge destination (home or other).

  5. Conclusion • Rehabilitation during hospital stay and discharge destination interacted to influence patients’ functional ability after hospital discharge in stroke patients and orthopedic patients. • Findings may be useful in deciding discharge destination for patients. • Transition-support program for rehabilitation patients discharged to their homes should be considered.
