  4. What you need in order to participate • Pay to Play $140 ($700 family cap) • Physical Examination Form &OHSAA Authorization Form (4 pages) • Emergency Medical Authorization Form • Athletic Information & Pledge Form • OHSAA Eligibility Checklist

  5. Academic Eligibility Requirements • OHSAA – Must receive passing grades in a minimum of five one credit courses or the equivalent (PE does not count) • FHSD – Must maintain no less than a 1.7 GPA (C- average) each quarter

  6. OHSAA Most Common Eligibility Issues • Transfer Rule • Entitled to 8 semesters of high school • May not turn 19 prior to August 1 of the current school year • May not participate in non-school team in the same season as your school team

  7. Athletic, Extracurricular and Co-Curricular Code of Conduct Participation in these activities is a privilege and not a right

  8. Athletic, Extracurricular and Co-Curricular Code of Conduct • In order to be successful, athletes must not use alcohol, steroids, or other performance enhancing or recreational drugs • The policy covers students in grades 7-12

  9. Medical Training Staff & Head Injuries • April Nierman & Mike Slemons, Certified Athletic Trainers • Athletes, coaches, parents, officials have a responsibility to report head injuries and know the signs and ramifications • IMPACT Testing • If a concussion occurs, athletes may not return to play until authorized to do so by a certified health professional

Transportation • Students participating in athletics are required to ride district transportation to and from contests. • If a student must ride to or from a contest with parents, the student must complete Alternate Transportation form in advance by parents and have athletic director sign. • Have students park in rear of building to allow front lot for evening guests

  12. Booster Fundraisers • Membership • Discount Card – sold in August • Kroger Gift Card • Booster Bash – reverse raffle ticket • Forest Green Waste Service • Concessions – parent required to work concessions two times per year

PARENT INVOLVEMENT IS THE KEY TO OUR SUCCESS • Be positive! • Be respectful to opposing teams & officials • Communicate scheduling problems • Take care of uniforms & equipment • Report any hazing/bullying immediately • Support the Booster fundraisers • Need one parent rep for each team • Remind students to lock personal items
