About the classes… • Adults 18 and over with chronic diseases (including: arthritis, diabetes, heart disease, and cancer.) • Taught by 2 trained leaders, professionals, or lay leaders who have a chronic disease themselves • Classes in community locations • Run for 2.5 hrs once weekly • Classes are grouped by disease • Highly interactive to facilitate learning

  11. Subjects Covered • Techniques to deal with fatigue, pain, & isolation • Appropriate exercise • Appropriate use of medication • Effective communication with family, friends, and health professionals • Nutrition

  12. Benefits • Improvements in: Exercise ability Cognitive Function General Health • Decreases in fatigue and disability • Increased Social Activity • Fewer Hospitals and Outpatient Visits These results have been proven to last at least 3 years!
