  4. WHAT ARE SHARED MEDICAL VISITS? A visit designed for a group of patients rather than individual provider-patient appointments

WHYDO PEOPLE LIKE SHARED MEDICAL VISITS? • Additional support for “me” from people like “me” (peer support) • Learn new things through education and discussion • Socialize, make new friends • Enhance relationship with their care team (patient centeredness) • Providers renew their joy of medicine It’s fun

What have we done to prepare? • 3 months ago • Determined the SMV theme: Cardiovascular disease • 2 months ago • Established a team, each member having an identified role and responsibility • 1 1/2 months ago • Found appropriate patients • 1 month ago • Set the date, time and duration of the group

  9. What do we prepare for every SMV? • Gather materials needed: • Name tags • Easels • Patient notebooks • Group handouts • Medical tools: BP cuffs, scale, vaccines & etc. • Solidify the agenda • Review documentation and charts • Huddle after the SMV

  10. What do we do at the first SMV with a new group? • Introductions • Description of a SMV • Confidentiality, HIPPA forms, medical waiver

  13. Why you were invited You have a risk for blood vessel disease (cardiac disease or stroke) Perhaps you have: • High blood pressure • Increased cholesterol • A history of heart problems • Less than optimal conditioning

  15. Let’s look at the heart(or Jeff’s anatomy) • Plumbing • blood vessels • valves • Electric: (the heart beat) • Pump: (Muscle)

  23. The pump is a muscle

  24. The heart muscle pumps blood through the body’s blood vessels

  25. So…why are the blood vessels so important? • The heart supplies ITSELF with blood • The heart and brain need more oxygen and nutrients than any other organs • There’s no rest __________________________________ (Ever had your thighs ache when climbing stairs?)

What happens if our blood vessels are affected • Your heart (you) might tire easy • You may have heart pain (angina) • If there is a real blockage, it can cause a heart attack or a stroke __________________________________________ But the damage takes many, many years and You can always stop the damage from worsening!
